JOB SUMMARY:Within the context of a patient, client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff. Reporting to the Patient Care Manager, receiving day-to-day supervision from the Coordinator (OT), FVG Rehabilitation Services, and receiving professional practice guidance from the Professional Practice Leader, Occupational Therapy, the Occupational Therapist supports the interdisciplinary program/team including rehabilitation assistants and peer support workers. Provides occupational therapy services to enable clients referred to the Foundry Vancouver Granville Clinic to achieve and maintain optimal health outcomes by helping achieve independent living and vocational goals. Performs assessments on client’s occupational performance utilizing an evidence-based approach; plans, develops and implements outcome oriented psychosocial rehabilitation interventions for clients; evaluates the effectiveness of the services; prepares and maintains records; participates in team/program meetings, quality improvement initiatives, and interdisciplinary research; and demonstrates safe and effective resource utilization and quality management. Collaborates with family members/significant others, program partners, inter-disciplinary team members, educational resources and other health care professionals/providers to achieve excellence in health services, education and evidence based evaluation. QUALIFICATIONS:Education, Training, and ExperienceBaccalaureate in Occupational Therapy and two (2) years’ recent related experience working with youth and young adults facing mental health and substance use issues in a community setting or an equivalent combination of education, training and experience. Current full registration as an Occupational Therapist with the College of Health and Care Professionals of BC (CHCPBC). Valid B.C. Driver’s License. Previous experience working in peer support and/or lived experience considered an asset. Assesses clients’ occupational performance issues, goals, interests, needs, safety and environmental supports and resources by utilizing recognized/applicable formal and informal occupational therapy, psychiatric rehabilitation, and/or physical assessments; reviewing records, gathering information relevant to the situation, interviewing the client, and determining suitability of services. 2. Plans outcome oriented occupational therapy services by collaborating with the client, caregivers/family and other health care providers; identifying client and therapist expectations for service and defining an approach and focus for service; establishing and prioritizing expected outcomes; discussing service implementation options; and determining resources and duration of service to meet identified client goals. 3. Implements occupational therapy services utilizing diverse intervention approaches by carrying out the plan, engaging the client in activities associated with occupational roles and the client’s goals, assessing and modifying the implementation, and identifying completion criteria to promote, develop, maintain and/or restore occupational potential. Acts as a group leader/co-leader in the provision of groups to meet the rehabilitation and psycho-educational needs of clients in the areas of personal life, leisure, education and work; and by supporting case managers with clients. May act as case manager for a small number of rehab-ready clients. Provides clinical direction to peer support worker and other support workers as appropriate. 4. Provides education and counselling to clients/families regarding the functional and rehabilitation issues of clients in the areas of personal life, leisure, education, work and housing; and provides information and guidance to caregivers, services providers, school staff and other community agencies.5. Evaluates occupational therapy services, including quality and outcomes, in collaboration with the client and family in keeping with the client’s goals by determining the purpose, method and criteria of the evaluation, implementing the evaluation, and analyzing and interpreting results to utilize results for service delivery and future planning of occupational therapy services. 6. Collaborates with interdisciplinary staff, rehabilitation assistants, peer support workers, health care professionals, school staff, clients/families/caregivers and other community resources in the identification and resolution of client/family/caregiver care issues, revision or termination of services, cultural and language barriers, ethical dilemmas, and coordination and integration of care, and makes referrals to other services providers, as appropriate. 7. Implements and supports peer support initiatives, training, data collection, review and reporting as required. 8. Supports youth employment initiatives including assessing client’s employment needs, identifying training, education and employment opportunities, meeting with employers, facilitating client employer relationships, providing feedback and coaching to clients for job interviews, educates employers on working with youth with mental health and/or substance use challenges.9. Maintains a variety of records including client files by gathering information, entering data into computerized databases and/or documenting information into approved formats according to the College of Occupational Therapists of BC practice standards. Notes client’s progress, changes to treatment plan, and records test/treatment results to provide evidence and rationale that support assessment findings and intervention plan. Compiles statistics on workload activities and summarizing information into written reports as requested.10. Participates in team/program activities required to enhance and optimize the delivery of care and to facilitate evidence-based practice. Promotes quality management principles for team/program operations, identifies processes/procedures that require improvement, and drafts/revises guidelines, policies and procedures for review. 11. Participates on committees as directed to promote optimal support for clients, to facilitate the resolution of issues, to represent the rehabilitation goals on regional committees, and to advocate for the goals and objectives of the team/program. 12. Participates in approved occupational therapy and interdisciplinary research and special projects in collaboration with designated personnel/team and other health care professionals, through methods such as collecting data, summarizing information, and/or providing verbal updates for review and further analysis by principal researcher, research team and/or project coordinator. 13. Maintains and updates own clinical knowledge within area of practice, and develops a plan in collaboration with designated personnel/team for professional development. Reviews progress to ensure that goals are achieved within established time frames.14. Provides input into the clinical and operational budget needs of the program.15. Performs other related duties as required.
