Baystate Health is seeking an Addiction Consultation Coordinator to join our Psychiatry Consultation Services at Baystate Medical Center in Springfield, Massachusetts. Our Addiction Consultation Coordinator supports inpatient patients across medical floors who are experiencing substance use and addiction-related needs, working closely with a multi-disciplinary team such as social workers, case management teams, providers, and community resources to connect patients with the services and support they need.

The Addiction Consultation Coordinator role offers the opportunity to make a meaningful impact by helping patients navigate addiction treatment and community resources, while building connections with local programs and developing a deeper understanding of available services. As the sole Addiction Coordinator, you will play an important role in supporting the consultation team and helping patients transition to appropriate levels of care, including Intensive Outpatient Programs (IOP) and Partial Hospitalization Programs (PHP).

The Addiction Consultation Coordinator Will:

  • Visiting and assisting inpatient patients throughout the facility as needed.
  • Connect patients with appropriate addiction treatment services, community resources, and follow-up care based on their individual needs.
  • Collaborate closely with Social Work, Case Management, and other members of the care team to support patients throughout their hospital stay and transition to ongoing services.
  • Help patients navigate available treatment options, including Intensive Outpatient Programs (IOP), Partial Hospitalization Programs (PHP), and other community-based resources.
  • Provide compassionate, respectful customer service and demonstrate strong bedside manner when engaging with patients and families.
  • Build and maintain relationships with community organizations and treatment programs, including opportunities to tour facilities and learn about their services, structure, and available programs.
  • Accurately and thoroughly document patient interactions, resources provided, and care coordination activities in the patient's medical record.
  • Serve as the primary Addiction Coordinator, helping ensure patients receive timely connections to appropriate addiction-related services and support.

Schedule Details

  • Job Type: Full-Time
  • Shift: Days
  • Hours: 40-hours per week
  • Schedule: Monday-Friday, 8am-4:30pm or 8:30am-5pm
  • Weekend Requirement: No weekends
  • Other Scheduling Requirements: No holidays

Required Skills  

  • High School Diploma, GED, or HiSET
  • 1 year of experience in a hospital or another medical setting.

Preferred Qualifications & Skills

  • 2 years working with individuals with Substance use disorders.
  • Minimum 2 years experience in multidisciplinary behavioral health or medical social work/care management setting.

Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
