We are seeking an experienced and dedicated Wound Care LPN to join our clinical team. This is an excellent opportunity to specialize in wound care while making a meaningful impact on residents’ health and recovery.

Position Summary
As a Wound Care Nurse (LPN), you will be responsible for providing specialized skin and wound care to residents under the direction of the physician, Director of Nursing, and/or Medical Director. You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes.
Key Responsibilities
- Provide direct wound care and skin treatments in accordance with physician orders
- Assess, monitor, and document wound progress and treatment effectiveness
- Assist in developing and modifying individualized care plans
- Collaborate with the interdisciplinary team to ensure optimal outcomes
- Educate staff on proper wound care techniques and prevention strategies
- Maintain accurate and compliant clinical documentation
- Ensure adherence to all regulatory, safety, and infection control standards
Qualifications
- Active, unencumbered LPN license in South Carolina
- Graduate of an accredited Practical Nursing program
- Current CPR certification required
- Ability to work 12-hour shifts
- Experience with PointClickCare (PCC) preferred
- Prior wound care experience strongly preferred
- Strong clinical judgment and attention to detail
- Passion for delivering compassionate, resident-centered care
