Description
Position Summary
This position is responsible for management and administration of day to day operations of the community based services program including: management and support of case management services; management of record keeping and data quality, supervision of assigned staff and interns, and compliance with all agency and funder requirements.
Essential Duties and Responsibilities
Case Management Oversight and Supervision
- Assist in landlord mediation when issues arise between tenants and landlords
- Assist case managers with challenging cases, accompanying staff to home visits as needed
- Oversee the Community Based Services staffing schedule and ensure coverage during absences
- Provide crisis intervention in mental health and/or medical emergencies
- Work with case managers to ensure goal achievement in the areas of housing, income and health
- Monitor program outcomes and ensure continuous quality improvement
- Engage in and provide clinical supervision in both group and individual formats
- Work with tenants to address issues in order to prevent eviction
- Manage and distribute petty cash, transit cards, and other resources for case managers
- With other Community Based Housing Lead Case Manager, host team meetings to ensure communication and staff development opportunities for community-based case managers
- Create systems and documents to improve compliance and time management for staff
Record Keeping and Data Quality
- Regularly review files to ensure timeliness and accuracy
- Work collaboratively with the Program Data Manager to ensure data quality
- Comply with HUD guidelines and participate in monitoring/site visits with HUD, the CoC, the Center for Housing and Health and other parties as need
Qualifications and Requirements
Education: Bachelor's Degree in Social Work or related field. Master degree preferred.
Licensure and/or Certification: Professional Licensure in the State of Illinois (LSW, LCSW, LCPC) or equivalent preferred.
Job Experience: Minimum 2 years of case management or social service experience. Minimum 2 years of supervisory experience.
Special Knowledge & Skills: Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, effective oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and own transportation required.
