Community Engagement Coordinator - Northwest New Mexico

The Community Engagement Coordinator serves as the primary community liaison and public representative for Axiom Care throughout Cibola, McKinley, San Juan and Sandoval Counties in New Mexico. This position is responsible for building and sustaining collaborative partnerships with Tribal Nations, rural communities, healthcare providers, behavioral health organizations, schools, local governments, and community-based organizations throughout Cibola, McKinley, San Juan, and Sandoval Counties in New Mexico. The coordinator identifies community needs, facilitates collaborative planning, and strengthens regional prevention, behavioral health, and community wellness initiatives through culturally responsive engagement and a strong understanding of the unique strengths and challenges of rural Northern New Mexico.

Serving as Axiom Care's primary field representative within the service area, the Community Engagement Coordinator maintains consistent communication with community partners, referral sources, and internal leadership. This position fosters collaborative problem-solving, documents outreach and engagement activities, gathers community feedback and data to inform program development, and cultivates sustainable partnerships that improve access to healthcare, behavioral health, prevention, recovery support, and other community-based services for underserved populations throughout the region. 

Full - time Exempt 

9:00AM - 5:00PM Occasional Nights and weekends as needed

Founded in 2012, Axiom Care is a Phoenix-based behavioral health organization that aims to ensure every Arizonan has access to compassionate, effective substance use treatment. Requirements

Duties/Responsibilities

• Serve as the primary community liaison and public representative for Axiom Care throughout Cibola, McKinley, San Juan, and Sandoval Counties in New Mexico.

• Establish, develop, and maintain collaborative partnerships with Tribal Nations, healthcare providers, behavioral health agencies, local governments, schools, community-based organizations, first responders, faith-based organizations, and other regional stakeholders.

• Build and strengthen referral networks that improve access to healthcare, behavioral health, prevention, recovery support, and community-based services.

• Conduct community outreach to increase awareness of Axiom Care's programs, services, and initiatives while encouraging collaboration and community participation.

• Maintain consistent, professional, and positive communication with community partners, referral sources, and stakeholders.

• Provide ongoing follow-up with existing and prospective partners to strengthen engagement, collaboration, and long-term relationships.

• Maintain regular communication with program leadership by providing timely updates regarding outreach activities, partnership development, community concerns, emerging opportunities, and project progress.

• Coordinate stakeholder meetings, listening sessions, coalition meetings, community forums, trainings, and collaborative planning activities.

• Represent Axiom Care at conferences, community events, health fairs, networking events, and regional meetings.

• Promote culturally responsive engagement practices that respect Tribal sovereignty, cultural traditions, and community priorities.

• Develop trusting relationships within rural and Tribal communities by understanding and responding to local healthcare, behavioral health, prevention, recovery, workforce, and access-to-care challenges.

• Collect qualitative and quantitative community data to support program planning, grant reporting, performance measures, and continuous quality improvement initiatives.

• Document outreach activities, stakeholder contacts, referrals, meeting outcomes, and community engagement efforts using designated tracking systems and databases.

• Identify barriers to collaboration and service delivery while working with internal staff and community partners to develop practical, sustainable solutions.

• Prepare reports and provide routine updates to leadership regarding outreach efforts, community engagement, partnership development, project outcomes, and emerging community needs.

• Monitor regional trends and identify opportunities to strengthen programs, expand partnerships, and improve access to services.

• Plan, coordinate, and deliver community presentations, educational workshops, and informational sessions.

• Organize and staff outreach booths, exhibits, and resource tables at conferences, health fairs, and community events.

• Assist with the planning and implementation of special projects, grant initiatives, and community engagement activities.

• Travel throughout Cibola, McKinley, San Juan, and Sandoval Counties as needed to support community outreach, meetings, and partnership development.

• Perform other duties as assigned.

Education and Experience:

• Minimum of 2 years in the field of behavioral health.

• Minimum 1 year in sales/business development.

• An Associates Degree in Business or Marketing preferred.

Physical Requirements:

• Prolonged periods of sitting at a desk and working on a computer.

• Must be able to lift up to 15 pounds at times. 

• Frequently required to spend multiple hours in a vehicle either as a driver or passenger.
