Bilingual Registered Nurse (RN) – Per Diem

Bilingual & willing to travel throughout Rhode Island (RI).

Job Summary: 

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.  

Essential Duties and Responsibilities:

Client Care 

• If needed by state law, completes an initial assessment of the client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). 

• Regularly re-evaluates client needs.  

• Initiates the plan of care and makes necessary revisions as client status and needs change. 

• Develops a care plan and includes the client and the family in the planning process. 

• Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act. (Skilled Nurse Only) 

• Counsels the client and family in meeting the client's needs. 

• Provides health care instructions to the client as appropriate per assessment and plan of care. 

• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client. 

Communication 

• Prepares clinical notes and updates the primary physician when necessary. 

• Documents any client-related updates. 

• Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required. 

• Communicates with community health-related persons to coordinate the care plan. 

Additional Duties 

• Participates in on-call duties as defined by the on-call policy. 

• Instructs, supervises, and evaluates personal care assistant care provided as required by state law. 

Qualifications and Skills:

• Graduate of an accredited school of nursing, with one (1) to two (2) years of recent acute care experience in an institutional setting. 

• Current, unrestricted licensure in the state and current CPR certification. 

• Bachelor's degree preferred, with one (1) year of home health care experience preferred. 

• Management experience not required; responsible for supervising personal care assistants. 

• Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills; nursing skills per competency checklist. 

• Licensed driver with an automobile in good working order, insured in accordance with organization requirements. 

Physical Demands:

• Prolonged or considerable walking or standing. 

• Able to lift, position, or transfer clients, and able to lift supplies and equipment. 

• Considerable reaching, stooping, bending, kneeling, or crouching. 

• Visual acuity and hearing sufficient to perform required nursing skills. 

Work Environment:  

• Home and community-based care settings; travel between clients' homes as required. 

• Participates in on-call rotation per departmental policy. 

Pay Range: This position is compensated on a per-visit basis, with rates of $65.00 for supervisory and 60-day visits and $75.00 for admission visits. Actual compensation will be determined based on a variety of factors, including, but not limited to, the candidate's relevant experience, qualifications, certifications, skills, and geographic location.
