Position Summary

The Physical Therapist evaluates residents, develops individualized treatment plans, delivers evidence-based therapy services, and collaborates with physicians, families, and the interdisciplinary care team to maximize resident outcomes and functional independence.

This role is ideal for a clinician who enjoys working in a skilled nursing and post-acute setting and is passionate about helping residents regain mobility, strength, balance, and confidence.

Key Responsibilities

• Evaluate residents to determine therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Improve residents' mobility, strength, endurance, balance, and overall functional independence.
• Supervise and direct Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Assist with training and development of Restorative Aides.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members regarding resident progress and discharge planning.
• Participate in care conferences, utilization review meetings, and rehabilitation team meetings.
• Provide educational in-services and training to facility staff.
• Complete accurate and compliant documentation in accordance with facility, state, federal, and payer requirements.
• Educate residents and families on home exercise programs, safety techniques, and fall prevention strategies.
• Recommend and coordinate durable medical equipment to support resident independence and mobility.
• Maintain a safe treatment environment and ensure proper equipment maintenance.
• Participate in quality improvement and performance enhancement initiatives.

Qualifications

Required

• Current California Physical Therapist (PT) license in good standing.
• Bachelor's degree in Physical Therapy (Master's or Doctorate preferred).
• Knowledge of therapy practices within a skilled nursing, rehabilitation, or post-acute setting.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
• Current CPR certification (if required by licensure).

Preferred

• Experience in skilled nursing, post-acute care, or long-term care.
• Experience with Net Health and PointClickCare documentation systems.
• Experience supervising PTAs, therapy aides, or students.

Physical Requirements

This position requires frequent standing, walking, lifting, transferring residents, and assisting with mobility activities. Candidates must be able to perform the physical demands associated with resident care and rehabilitation services in accordance with safe patient handling practices.
