Affirmed Home Care – Per Diem Assessment RN North Jersey

Registered Nurse – Per Diem Assessments and On-Call Visits:

Affirmed Home Care is seeking qualified and dependable Registered Nurses to perform per diem assessments and on-call nursing visits in patients’ homes. Serving all counties in North Jersey. 

This opportunity is ideal for Registered Nurses interested in earning additional compensation while continuing in their current field-based or full-time position. Visits may be accepted according to the nurse’s availability.

Job Responsibilities:

• Complete start-of-care assessments

• Conduct supervisory nursing visits

• Perform patient reassessments

• Complete post-hospitalization visits

• Provide on-call nursing visits as assigned

• Assess patients’ clinical conditions and care needs

• Report changes in patients’ conditions to the clinical team

• Complete all required visit documentation accurately and on time

• Maintain patient confidentiality and follow HIPAA requirements

• Comply with Affirmed Home Care policies and applicable nursing regulations

Qualifications:

• Active and unrestricted Registered Nurse license

• Previous nursing experience required

• Home care or field nursing experience preferred

• Strong assessment and documentation skills

• Ability to work independently in patients’ homes

• Excellent communication and time-management skills

• Reliable transportation

• Current CPR or BLS certification
