What we are looking for:

The Virtual Safety Monitoring program is dedicated to remotely monitoring and ensuring the safety of patients. The VSM has enhanced patient care, prioritizing patient safety and well-being by addressing gaps in traditional healthcare systems, ensuring consistent and attentive care at all times.

Reporting to the Patient Care Manager, Access and Flow, as a member of the Access and Flow Command Center team, the Virtual Safety Monitoring Technician plays a vital role in providing support and assistance to patients through remote communication channels by providing continuous observation & monitoring to patients with safety concerns. 

This role requires strong communication, technological proficiency, keen observation and critical-thinking skills, as well as the ability to remain calm, focused and responsive in a high-pressure environment.

In this role, you will:
 

  • Monitor patients remotely using advanced telemonitoring technology to promote patient safety and well-being.
  • Maintain continuous observation of patients with identified safety concerns and recognize changes in behaviour or condition.
  • Communicate with patients through remote channels, providing reassurance, support and assistance as required.
  • Communicate changes in patient behaviour or condition to the appropriate clinical teams and escalate concerns promptly when required.
  • Work closely with interprofessional healthcare teams to support timely response, de-escalation and resolution of safety concerns.
  • Complete accurate and timely documentation of patient observations and interactions.
  • Support the assessment and triage of patients for suitability for the Virtual Safety Monitoring program.
  • Participate in continuous improvement initiatives that support program development, innovation and patient safety.
  • Manage multiple priorities effectively while monitoring up to 12 patients simultaneously.
  • Use Microsoft Office and other technology platforms confidently and efficiently.

  • Strong observation skills and exceptional attention to detail.
  • Excellent problem-solving, critical-thinking and decision-making abilities.
  • Strong conflict management, crisis management and de-escalation skills.
  • The ability to recognize risks, determine when escalation is required and collaborate effectively with clinical teams.
  • Strong technological aptitude and the ability to quickly adapt to different software platforms.
  • The ability to prioritize competing demands and manage multiple responsibilities in a fast-paced environment.
  • The ability to work effectively both independently and as part of an interprofessional team.
  • A demonstrated commitment to patient- and family-centred care.
  • A commitment to ongoing professional development and continuing education.
  • Additional language skills aligned with the hospital's patient population are considered an asset.

Qualifications:

  • Secondary School Diploma required.
  • Bachelor’s degree in health or related areas is preferred.
  • 2–4 years of previous experience in a customer service or front-facing position required.
  • 1–3 years of previous experience working in a healthcare environment preferred.
  • Previous experience with Meditech is considered an asset.
  • Training or certification in Gentle Persuasion Approach (GPA), Dementia Ability Methods, P.I.E.C.E.S. or Crisis Prevention Intervention is considered an asset.
  • Proficiency with Microsoft Office applications and other technology platforms.
  • Demonstrated ability to communicate effectively in English, both verbally and in writing.

Work Schedule:

 

D, E, and N shifts consisting of 8- and 12-hour shifts, including days (7:00 a.m.–3:00 p.m. and 8:00 a.m.–8:00 p.m.), evenings (4:00 p.m.–12:00 a.m.), and nights (8:00 p.m.–8:00 a.m.), Monday to Sunday, including statutory holidays.
