Location - PACE New Jersey
Hours- Monday - Friday 8-430
Shift - First Full Time
Life Specialist to provide personal care, support, and wellness services for participants in a PACE (Program of All-Inclusive Care for the Elderly) environment.

Key Responsibilities
- Provide personal care assistance, including bathing, grooming, toileting, dressing, eating, and mobility support.
- Monitor participants' health status and report changes in physical, emotional, or behavioral conditions.
- Assist with clinic activities, including taking vital signs, escorting participants to exam rooms, preparing participants for care, and maintaining exam room supplies.
- Support participant safety by identifying hazards and responding appropriately to emergencies.
- Assist with recreational activities, meal service, housekeeping, and participant engagement.
- Escort participants to medical and other appointments as needed.
- Maintain accurate documentation and participant confidentiality.
- Collaborate with the interdisciplinary care team to support participant-centered care.
Qualifications
- High school diploma or equivalent required.
- Active Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification in the state of employment required.
- Minimum of one year of experience working with frail or elderly populations, or willingness to complete specialized training upon hire.
- Experience in long-term care, senior services, or community-based geriatric programs preferred.
- Reliable transportation required.
