Bluebird Health Clinical Liaison FT

JOB DESCRIPTION SUMMARY 

The Clinical Liaison is responsible for managing on-site Healthcare Clinic and works collaboratively with the members of the team to help meet positive patient care outcomes and enhance overall company profile.  Facility Liaison makes independent decisions regarding the clinic.  Facility Liaison will manage aides and caregivers at the clinic. 

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 

• Manages all aspects of the Healthcare Clinic such appointments and inventory. 

• Performs assessment on resident in the facility 

• Evaluates residents for services such as hospice, HH and homecare. 

• Interacts with residents, physicians and other providers to assure optimal outcomes in patient care. 

• Prepare and submit end of day reports to Clinical Management and Business Development Manager. 

• Manages and delegates to the aide/caregiver(s). 

• Provides direct patient care as defined in Idaho Nurse Practice Act. 

• Participates in personal and professional growth and development. 

• Actively participate with the IDG in collaborating to ensure the patient’s plan of care is kept current and any identified issues are addressed.  

• Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.   

• Provides community outreach and education. 

• Assist office staff as needed with going to Physician’s offices to obtain signatures, drop off IDG updates, etc.  

• Assists in development and implementation of a marketing plan staying within established timetable.

POSITION QUALIFICATIONS 

• Graduate of an accredited practical nurse or vocational nursing program. 

• Has three years nursing experience.  Community/home health or medical/surgical experience is preferred. 

• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.  

• Complies with accepted professional standards and practice 

• Demonstrates good verbal and written communication, and organization skills. 

• Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order. 

• Possesses and maintains current CPR certification.
