Home Health Physical Therapist – Hospital Affiliated | PSLF Eligible | $5K Sign-On

- Compensation: $93,000 - $99,000 Base Salary (Based on experience)
- Pay-Per-Visit (PPV) bonus structure: Earn additional premium pay for any visits completed over the 30-unit weekly target
- $5,000 Sign-On Bonus
- Standard $0.63/mile reimbursement for travel (excluding first and last drive)
- Schedule Control: Dedicated team schedules your start-of-cares, allowing you to route the rest of your day efficiently
- Caseload: Post-surgical, total joints, COPD, debility, and general medical
- Weekend on-call rotation (1 weekend per month requirement)
- Paid Time Off (PTO) = 18 days annually

About the Physical Therapist Position: Our ideal candidate will possess the heart of a servant leader and have a passion for patient-centered care. Education of the patient is the number one priority in helping them realize how their body will react to treatment and therapy.
Physical Therapist Qualifications & Requirements:
- Bachelor's, Master's, or Doctorate of Physical Therapy from an accredited program
- Minimum 1-year of inpatient or home health experience required
- Ability to independently manage your schedule and a metric-driven caseload
- Team player who collaborates well across disciplines
- Servant leader mindset
