Job Summary and Responsibilities
As a Registered Nurse (RN), you will be a pivotal healthcare professional, delivering compassionate, high-quality care that truly impacts our patients' well-being and recovery.Every day, you will leverage your expertise to provide individualized, comprehensive care, making critical assessments, performing skilled procedures, and meticulously implementing patient care plans. - Assess and develops a plan of care.
- Provides case management and documents skilled care, treatments and diagnostic procedures.
- Completes and documents initial evaluation of assigned patients.
- Assesses patient status and skilled nursing needs on admission and each subsequent visit.
- Prepares recertification, discharge summaries and all federally required data entry and/or forms for Medicare.
Job Requirements
Required
- Oregon RN License
- AHA BLS within 30 days
- Two years acute care nursing experience
- Associate's Degree in Nursing
- Oregon Driver's License and Auto Insurance
Preferred
- One year Hospice experience
