St John of God Midland Private Hospital is a brand new standalone private hospital delivering modern medical and surgical care for people living in Perth’s eastern suburbs and the Wheatbelt.
The 123-bed purpose-built hospital located within the historic Midland railway workshops precinct opened in August 2026.
Designed with patients, families and clinicians in mind, the hospital provides modern facilities, enhanced care capabilities and a welcoming environment that supports health and healing.

About the role
We are seeking a Discharge Coordinator on a fixed term full-time basis from 30th November 2026 to 1st August 2027.
The Discharge Coordinator is responsible for the delivery of patient centred care within their area of specialty to achieve optimal patient outcomes within allocated resources whilst supporting and promoting the Mission and Values of St John of God Health Care (SJGHC).
In collaboration with the Nurse Managers and Director of Nursing the Discharge Coordinator will be responsible for improving and maintaining standards, developing solutions and delivering outcomes consistent with service and organisational imperatives and targets.
This includes the provision of leadership, clinical performance monitoring, policy review and development, development and delivery of clinical education, implementing changes to clinical practice and initiating research and quality improvement activities.
Key Responsibilities
- Conducts nursing/midwifery practice in accordance with Nurses and Midwives Board of Australia (NMBA) Competency Standards, regulations and Scope of practice.
- Maintains advanced clinical skills and provides direct clinical care in the area of specialty practice.
- Incorporates Evidence Based Nursing Practice into patient care and leadership responsibilities.
- Acts as a resource in planning, implementing and evaluating care for patients.
- Facilitates the provision of advanced and complex patient care within the area of specialty, working in partnership with the patient, their family, the interdisciplinary team and external service providers.
- Provides specialised training and education resources to the broader multidisciplinary team to support best practice care delivery in the area of specialty practice.
- Evaluates the quality of practice in area of specialty using key performance/best practice indicators, national and international benchmarking.
- Identifies unsafe care practices and implements corrective action in collaboration with others as required.
- Maintains an intentional and dedicated focus on patient safety, person-centred care and experience at all times.
Your skills and experience
You will hold registration with the Nursing and Midwifery Board of Australia as a Registered Nurse/Midwife and demonstrate significant knowledge, experience and leadership in this specialty. You will have a demonstrated knowledge and application of quality improvement initiatives and well developed interpersonal and communication skills along with skills in clinical teaching.

- Salary: $129,203 per annum plus 12% superannuation
- 8 month fixed term full-time position
