We are looking for a compassionate Hospice Chaplain to serve patients and families throughout the Springtown and Azle area. Whether you're looking for the stability of a Full-Time position or the flexibility of PRN, this is an opportunity to use your gifts to make a meaningful difference.
Our Chaplains meet people where they are, providing a listening ear, comfort, encouragement, prayer when desired, and spiritual support that respects each person's individual beliefs and wishes.
What You'll Do
- Provide spiritual and emotional support to hospice patients and their families
- Complete spiritual assessments and individualized plans of care
- Provide supportive visits, counseling, prayer, and spiritual care based on patient and family preferences
- Respect and support patients of all faiths, beliefs, cultures, and backgrounds
- Support patients and families experiencing anticipatory grief and loss
- Collaborate with nurses, social workers, aides, physicians, and other members of the hospice team
- Participate in interdisciplinary team (IDT) meetings
- Coordinate with community clergy and spiritual resources when requested
- Complete timely and accurate documentation in the EMR system
What We're Looking For
✔ Education and/or training appropriate for providing pastoral or spiritual care
✔ Previous chaplain, ministry, pastoral care, or spiritual care experience
✔ Hospice or healthcare experience preferred
✔ Compassionate and respectful approach to individuals with diverse beliefs and backgrounds
✔ Strong listening, communication, and interpersonal skills
✔ Ability to work independently while collaborating with an interdisciplinary team
✔ Reliable transportation and willingness to travel throughout the Springtown/Azle service area
Hospice experience is preferred but not required. We're looking for someone with a genuine heart for serving patients and families during the end-of-life journey.
