Aviary Health's mission is to empower everyone to live a long, full, and optimal life. We're redefining the future of heart and lung health through our Intensive Cardiac Rehabilitation (ICR) and Pulmonary Rehabilitation programs - partnering with health systems and medical groups nationwide to deliver virtual and in-person clinical programs.

We're looking for an operationally-oriented Manager, Partner Implementations to join our growing Partnership Success team and lead the launch of new health system and medical group partnerships. Reporting to the Director of Implementations, you will own end-to-end implementation project management for a portfolio of partners - from signed contract through go-live and post-launch stabilization.

This role sits at the intersection of healthcare operations, technology, and relationship management. You'll coordinate across internal functions (Operations, Clinical, Product, Engineering, Insurance, and Enrollment) and serve as the single point of contact for partners navigating complex EHR integrations, clinical workflow design, billing setup, and referral pipeline activation.

You will work alongside two existing Implementation Managers and play a critical role in scaling our implementation methodology as Aviary expands across new markets and service lines.

Responsibilities

In this role, you will:

• Own partner implementations end-to-end - serve as the primary project manager both internally and externally for active client implementations, managing project plans, timelines, risk tracking, and stakeholder communication
• Coordinate cross-functionally across Operations, Clinical, Product, Engineering, Insurance, and Enrollment teams to stand up and launch Aviary's programs with new health systems and medical groups
• Be the single point of contact for partners during implementations, building trust with clinical leaders, IT teams, billing departments, and executive sponsors to achieve launch targets
• Lead EHR integration workstreams - partner with Engineering and IT counterparts to manage interface builds (ADT, SIU, referral, scheduling), Epic/Athena/eCW workflows, and testing cycles through go-live
• Drive clinical and operational readiness - coordinate implementation training, billing/claims setup (855B applications), and insurance verification workflows
• Manage pipeline activation - work with partners to configure referral workflows, proactive outreach campaigns, and patient engagement strategies to maximize enrollment at launch
• Travel to partner sites for go-lives, onsite training, and relationship-building (up to 40-50% monthly during active implementations)
• Evolve Aviary's Implementation Playbook - identify improvements to standardize quality, efficiency, and effectiveness across implementations; create and maintain documentation for key workstreams
• Monitor post-launch performance - track core metrics (referrals, enrollment, census, charges), present data to partners, and troubleshoot operational issues in the weeks following go-live
• Support QBRs and transitions - prepare materials for Quarterly Business Reviews and lead structured handoffs from Implementation to Account Management

Requirements for the Role:

Must-haves:

• Bachelor's degree
• 3-5 years of professional experience in implementation project management, healthcare consulting, or healthcare operations
• At least 2 years working directly with hospitals, health systems, or medical groups as clients or partners
• Experience with EHR systems (Epic, Athena, eClinicalWorks) — particularly integration, interface builds, or clinical workflow configuration
• Demonstrated experience launching a new product, program, or partnership — not just maintaining existing ones
• Strong project management skills: you can run a complex, multi-workstream project plan and hold internal and external stakeholders accountable to milestones and deadlines
• Effective communicator who can engage, inspire, and build credibility with clinical leaders, IT teams, and C-suite executives
• Comfortable operating in a fast-paced, less-structured startup environment where no perfect playbook exists
• Highly detail-oriented, extremely organized, and results-driven; exceptional at creating process and structure
• Excellent collaboration and time management skills
• Effective communication skills: the ability to be direct, logical, and succinct
• Experience meeting deadlines, keeping many balls in the air, and prioritizing work in a fast-paced, dynamic environment
• Desire to improve the status quo
• Willing to travel up to 50% during active implementation periods and live near a major airport
• NYC-based and willing to work in our Tribeca office 2+ days/week strongly preferred

Strong-to-haves:

• Background in management consulting or healthcare IT consulting
• Familiarity with healthcare billing, credentialing, or insurance verification processes
• Experience working at a digital health startup or health tech company selling to health systems
• Comfort with data/analytics tools (Metabase, Excel, MixPanel) to track and present partner metrics
• Experience supporting hospital-based implementations with complex IT, compliance, and governance requirements
