Pay Range: $22.00–$24.00 per hour, based on experience

Willow Creek Healthcare Center is seeking an energetic, compassionate, and creative Full-Time Activities Director to lead our resident activities program. The Activities Director plans, develops, and oversees resident-centered programs designed to meet each resident’s interests and physical, cognitive, emotional, social, and psychosocial needs.
Essential Duties and Responsibilities
- Plan, develop, implement, evaluate, and direct the facility’s resident activities program.
- Create a varied monthly calendar that includes recreational, social, educational, cultural, spiritual, and wellness programs.
- Provide individual, small-group, large-group, and in-room activities.
- Develop specialized programming for residents who are bed-bound, isolated, cognitively impaired, or unable to participate in traditional group activities.
- Complete comprehensive activity assessments, care plans, progress notes, and other required documentation.
- Ensure progress notes clearly describe the services provided and each resident’s response.
- Participate in care conferences, discharge planning, and the development and implementation of individualized care plans.
- Encourage residents to pursue hobbies, crafts, reading, and other self-directed interests.
- Provide accessible materials, including audiobooks, large-print resources, or Braille materials when appropriate.
- Plan resident outings and arrange transportation for activities, appointments, and discharges when necessary.
- Recruit, schedule, train, supervise, and evaluate Activities Department employees and volunteers.
- Maintain accurate attendance records, calendars, departmental reports, and program documentation.
- Communicate effectively with residents, families, employees, community partners, support personnel, and regulatory agencies.
- Participate in department head, general staff, quality assurance, infection control, safety, marketing, resident council, and family council meetings.
- Participate in community planning related to facility programs and resident needs.
- Assist the Quality Assurance and Assessment Committee with improvement plans and corrective actions.
- Maintain compliance with federal, state, local, and facility standards governing resident activities.
- Participate in regulatory surveys and assist with developing and implementing plans of correction when needed.
- Maintain a clean, organized, safe, and welcoming Activities Department.
- Protect residents’ privacy and maintain the confidentiality of all health information.
- Perform additional duties as assigned.
Qualifications
- High school diploma or equivalent required.
- Current Activity Director certification required.
- One year of experience in a skilled nursing or long-term-care facility preferred.
- Previous activities, recreation, senior living, healthcare, or supervisory experience preferred.
- Strong knowledge of resident-centered programming, assessments, care planning, and documentation.
- Creative, energetic, patient, and compassionate leadership style.
- Excellent communication, organization, presentation, and time-management skills.
- Ability to supervise employees, manage multiple priorities, and solve practical problems.
- Ability to read and understand technical procedures, regulations, and facility policies.
- Basic computer skills and experience with electronic documentation systems preferred.
- Dependable attendance and flexibility to support special events, weekends, or holidays as needed.
Physical Requirements
This position requires frequent standing, walking, reaching, pushing, pulling, talking, and hearing. The employee must be able to lift or move up to 50 pounds frequently and occasionally climb, balance, stoop, kneel, crouch, or crawl.
