Project Manager Family Nurse Practitioner (Grant-Funded)

Please Note: This is a grant-funded position currently funded through June 30, 2027.

This position includes travel to Los Angeles as needed

About the Role

The Project Manager Family Nurse Practitioner (PMFNP) provides both clinical and operational leadership for the Dr. Gloria J. McNeal Nurse Managed Clinic's community-based healthcare programs. The selected candidate will oversee clinic operations, supervise healthcare personnel, support grant initiatives, and deliver direct patient care to underserved populations.

This role serves individuals experiencing health disparities, including people experiencing homelessness and other at-risk populations, while also supporting healthcare services provided in community settings, including elementary schools.

Candidates must possess Family Nurse Practitioner (FNP) certification and have experience providing care across the lifespan.

Location & Travel

  • Candidates must reside in San Diego, CA
  • Frequent travel between clinic and community sites within the candidate's assigned region is required
  • Travel expenses are reimbursed
  • Ability and willingness to travel up to 75% to support clinic operations, outreach initiatives, and grant-related activities

What You'll Do

Clinical & Community Care

  • Deliver primary care services to underserved and vulnerable populations
  • Provide healthcare services to individuals experiencing homelessness and other at-risk populations
  • Support community-based healthcare initiatives throughout Los Angeles and San Diego
  • Deliver care within mobile, community, and school-based settings
  • Collaborate with interdisciplinary healthcare teams to improve patient outcomes
  • Monitor patient care plans and support continuity of care

Clinic Operations & Leadership

  • Manage day-to-day operations across multiple clinic locations
  • Oversee clinic workflows, scheduling, and patient-centered service delivery
  • Supervise Nurse Practitioners and Community Health Administration staff
  • Support billing, credentialing, compliance, and quality improvement initiatives
  • Manage Electronic Health Record systems and operational processes
  • Ensure culturally responsive and high-quality care delivery

Grant & Program Management

  • Support all grant-funded initiatives and compliance requirements
  • Collaborate with internal and external stakeholders to achieve project goals
  • Assist with federal reporting, outcome tracking, and data collection
  • Support budget monitoring and grant deliverables
  • Partner with community organizations to expand access to care

Community Outreach

  • Build relationships with community agencies, schools, and service organizations
  • Represent the clinic at community events and health fairs
  • Support initiatives that increase healthcare access for underserved populations

Required Qualifications

Education

  • Master of Science in Nursing (MSN) required
  • Doctoral degree in Nursing, Healthcare, or related field preferred

Licensure & Certification

  • Family Nurse Practitioner (FNP) certification required
  • Current and unencumbered Advanced Practice Registered Nurse (APRN) Certification and Registered Nurse (RN) Licensure required.
  • Active California furnishing number, National Provider Identifier (NPI), and DEA registration number required.
  • Maintain current national and state board certification as a Nurse Practitioner from an approved organization such as:
    • American Nurses Credentialing Center (ANCC)
    • American Academy of Nurse Practitioners (AANP)
    • National Certification Corporation (NCC) for Obstetrics, Gynecologic, and Neonatal Nursing Specialties

Experience

  • Minimum 5 years of healthcare leadership or management experience
  • Experience serving underserved, vulnerable, or historically marginalized populations
  • Experience managing healthcare programs, clinical teams, or community-based services
  • Experience with grant-funded healthcare programs preferred
  • Pediatric experience required, as the role includes providing care within elementary school settings
  • Experience working with communities experiencing homelessness strongly preferred

Preferred Qualifications

  • Federal grant management experience
  • Nurse faculty or preceptor experience
  • Experience working with community-based organizations and public health initiatives
  • Experience supporting behavioral health and primary care integration
