Licensed Social Worker (LSW) – Behavioral & Residential Services | Toledo, OH
We are partnering with a well-established behavioral health and residential services organization in Toledo, Ohio to find a compassionate and skilled Licensed Social Worker (LSW) to join their dedicated clinical team. This is an excellent opportunity for a licensed social worker who is passionate about making a genuine difference in the lives of adults and youth navigating mental health, behavioral, and life challenges.
Position Details
- Setting: Behavioral Health / Residential Services — residential group home, behavioral health facility, and community settings
- Patient/Client Population: Adults and youth — mental health, behavioral health, and residential services
- Schedule: Full-Time | Monday – Friday, typically 9:00 AM – 5:00 PM | Evening and weekend availability may be required based on client need
- Reports To: Clinical Director or Executive Director
- Location: Toledo, OH (some services conducted in community settings)
- Compensation: Competitive, commensurate with experience
- Start Date: ASAP
About the Role
The Licensed Social Worker provides behavioral, emotional, and mental health support for clients across residential and community settings. You will evaluate clients, develop individualized treatment plans, connect clients with community resources, collaborate with other care providers, and respond to crisis situations. This role requires a dynamic clinician who can navigate complex family and community systems while maintaining strong therapeutic relationships and compliance with applicable Ohio regulatory standards.
Key Responsibilities
- Provide behavioral, emotional, and mental health support to adults and youth clients
- Conduct evaluations and develop comprehensive treatment plans, discharge plans, and progress notes; review and revise plans as clinically appropriate
- Serve as an advocate for clients, ensuring they meet their academic, personal, and health potential
- Assist clients managing mental health disorders, substance use, and co-occurring conditions — develop appropriate treatment and social rehabilitation plans
- Connect clients with community resources including prescriptions, DME, housing, health programs, and public/private services
- Develop effective working relationships with clients, families, physicians, team members, and community partners
- Respond appropriately to crisis situations with sound clinical judgment
- Complete all required daily documentation in accordance with agency and accrediting body guidelines
- Collaborate with the Administrator on agency compliance with ODJFS, ODCYS, CARF, OHMAS, and applicable Ohio Administrative Code regulations
- Participate in committees and Performance Improvement Projects
- Maintain compliance with all State of Ohio health policies and procedures
Requirements
- Bachelor's degree in Social Work — required; MSW preferred
- Valid Ohio LSW license in good standing — required
- Minimum 6 months of therapy or assessment-related experience — required
- Valid Ohio driver's license with clean driving record — required (position requires driving your own vehicle)
- Current auto insurance — required
- Must pass all required background checks
- First Aid and CPR certification — required (or obtainable upon hire)
- Proficiency with Microsoft Office suite and Electronic Health Records (EHR) systems
- Ability to react with sound judgment in crisis situations
- Strong communication, empathy, and team collaboration skills
- Bilingual in English and Spanish — a plus
- Comfort with technology, change, and working in a growth-oriented organization
