Job Description:
Home Health Licensed Practical Nurse (LPN)
PRN | Beverly to Peabody
$32–$38 per hour, based on experience

What You’ll Do
Provide skilled nursing care under the direction of an RN and physician
Follow individualized plans of care and physician orders
Monitor patients and report changes to the RN Case Manager
Educate patients and caregivers on medications, treatments, and self-care
Complete accurate and timely clinical documentation
Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits
What You’ll Need
Current Massachusetts LPN license
Graduate of an accredited nursing program
At least one year of home care experience
Valid driver’s license, reliable transportation, and current auto insurance
Ability to travel throughout Malden, Lowell, and surrounding communities
Home health or hospice experience preferred
Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
