At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Care is delivered directly on the street, encampment, or shelter.

Job Purpose:

Provide compassionate, high-quality medical care directly to individuals experiencing homelessness while serving as the on-the-ground clinical lead for the launch and stabilization of a new Healthcare in Action market. The Nurse Practitioner, HIA (Interim Lead, New Market) assesses, diagnoses, and treats patients in nontraditional settings and provides regional leadership for clinical quality, operational readiness, financial and productivity stewardship, care model implementation, team build-out, compliance readiness, and external partnership development. The role works in close collaboration with enterprise medical, clinical operations, finance, revenue cycle, compliance, human resources, and executive leadership resources.

Responsibilities:

Direct Clinical Care

Provide comprehensive, patient-centered assessment, diagnosis, treatment, prescribing, preventive care, and chronic disease management in street-based and other nontraditional settings, consistent with applicable scope-of-practice requirements.

Care Planning and Coordination

Develop, implement, and update patient-centered care plans in collaboration with patients, peer navigators, physicians, and interdisciplinary partners; initiate referrals and coordinate with internal and external healthcare resources to address medical and social needs.

New Market Clinical Leadership

Serve as the visible, on-the-ground clinical leader for new market launch and ongoing operations. Translate enterprise  priorities into clear regional actions, proactively identify gaps and unclear ownership, research and recommend solutions, escalate material risks, and drive cross-functional work through completion.  Provide concise, timely updates on regional performance, risks, decisions, resource needs, and recommended actions.

Clinical Quality and Outcomes

Own the regional quality dashboard and review cadence; identify metric gaps, outliers, and underperforming patient cohorts; lead documented quality reviews; establish corrective or improvement plans; and drive measurable progress against assigned clinical, access, care coordination, and housing-related goals.

Financial and Productivity Stewardship

Participate actively in financial and productivity reviews; develop working knowledge of the regional staffing model, cost drivers, productivity expectations, and revenue realization levers; communicate targets to the team; and develop corrective actions when performance deviates from plan, with support from Finance and Revenue Cycle.

Care Model Transformation

Lead regional execution of assigned enterprise care model initiatives, including adoption of standardized clinical workflows, electronic health record dashboards, documentation standards, billing integration, and coding practices. Track adoption, remove barriers, and connect frontline clinical realities with operational execution.

Team Build and People Leadership

Partner with Human Resources and Talent Acquisition to execute the approved hiring plan, participate in selection of clinical and operational talent, support timely onboarding, establish role expectations, and create a cohesive, accountable regional team. Provide direct or functional supervision, coaching, and performance support as assigned.

External Partnerships and Market Development

Build and sustain productive relationships with priority external stakeholders, including healthcare systems, community providers, referral partners, housing and social service organizations, and governmental or public-sector partners. Represent HIA professionally and advance agreed quarterly partnership objectives, with executive support as appropriate.

Compliance, Audit Readiness, and Safety

 Maintain clinical, regulatory, privacy, safety, documentation, and infection-control standards. Establish regional audit-readiness practices and ensure identified issues are promptly escalated and remediated.

All other duties as assigned.

SUPERVISORY RESPONSIBILITIES

Provides direct and/or functional leadership for clinical and non-clinical regional team members as assigned during the interim period. Supervisory scope, reporting relationships, and decision authority will be documented in the approved interim appointment and organizational structure.

Qualifications and experience:

Minimum Education

  • Master’s degree from an accredited Nurse Practitioner program.

Required Certifications and Licenses

  • Current, unrestricted Nurse Practitioner licensure and all prescribing or furnishing authorities required for practice in the state of assignment.
  • Current national board certification in an applicable population focus, as required for the position and state of practice.
  • Current Basic Life Support certification; additional certifications as required by clinical assignment.
  • Valid driver’s license and ability to meet organizational driving requirements.

Required Experience

  • Demonstrated success operating in a new, evolving, or loosely defined environment, independently researching unfamiliar issues, identifying what is needed, proposing practical solutions, and driving work forward with limited direction.
  • Demonstrated experience delivering direct patient care as a Nurse Practitioner.
  • Demonstrated ability to lead interdisciplinary work, make sound clinical decisions, and coordinate across clinical and operational stakeholders.
  • Experience using electronic health records and clinical data to support patient care, documentation, quality improvement, and operational decisions.

Preferred Experience

 Experience launching or stabilizing a clinical program, market, site, or team, including hiring, operational readiness, quality improvement, compliance, financial performance, and external partnership development.

PHYSICAL AND TRAVEL REQUIREMENTS

  • Works in field-based, outdoor, community, and other nontraditional care settings; complete the applicable non-office ADA worksheet for the position.
  • Ability to safely assist patients as needed during assessment and treatment, consistent with the approved physical requirements of the role.
  • Extensive local driving is required. Out-of-town travel may be required based on market-launch and business needs.
  • Work mode is primarily field-based and community-facing, with remote and administrative work as business needs permit.
