Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Licensed Clinical Social Worker (LCSW) in Fort Plain, New York.

POSITION OVERVIEW
• Outpatient behavioral health role providing counseling and therapeutic services for individuals and families
• Flexible scheduling options including hybrid scheduling and four 10-hour shift opportunities
• Collaborative interdisciplinary care environment with full clinical and administrative support staff
• Mission-driven behavioral health organization focused on long-term patient care and support
• Upstate New York community offering small-town living, outdoor recreation, and strong community connections

COMPENSATION
• Base Salary: $70,000 – $80,000
• Student loan repayment eligibility up to $50,000

SCHEDULE
• Monday through Friday schedule
• 8:00 AM – 5:00 PM
• Option for four 10-hour shifts
• One evening weekly until 7:00 PM
• Hybrid scheduling available

ROLE
• Provide counseling and therapeutic services to individuals and families
• Work with patient populations age 5 and older
• Maintain documentation and treatment planning within electronic health record systems
• Collaborate with interdisciplinary care teams to coordinate patient support
• Support continuity of care and long-term behavioral health outcomes

Requirements

• Clear and active New York LCSW license REQUIRED
• Strong verbal and written communication skills REQUIRED
• Comfortable working with patient populations age 5 and older REQUIRED
• Ability to collaborate within interdisciplinary care environment REQUIRED
• Experience working within electronic health record systems REQUIRED
• Strong patient-centered and team-oriented care approach REQUIRED
