Responsibilities
As a Clinical Community Liaison, You will:
- Meet with hospitals, nursing homes, and community providers to promote awareness of hospice care.
- Help identify patients who may benefit from hospice services and assess their eligibility.
- Coordinate care between referral sources and our hospice admission team.
- Support patients and families through the transition from hospital or home to hospice care.
- Track and understand local healthcare needs, referral trends, and competitors.
- Participate in outreach events and support the marketing team with planning and strategy.
Qualifications
- Completion of an accredited Registered Nurse (RN) program.Completion of a Registered Nurse or Licenses Practical Nurse accredited program with an unrestricted state nursing license.
- Three years of nursing experience required
- One year required in hospice or home health
- Hospice experience preferred.
- Must hold an active and unrestricted state nursing license.
- Demonstrate effective marketing skills and the ability to engage tactfully with customers, community members, and the hospice clinical team.
- Exhibit strong communication, negotiation, and public relations skills.
- Display autonomy, organizational ability, assertiveness, flexibility, and cooperation in fulfilling job responsibilities.
- Apply knowledge of disease processes and conduct comprehensive medical record reviews.
