What You'll Do
- Perform comprehensive patient assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services, medication management, wound care, and patient education
- Monitor patient progress and communicate changes to physicians and the interdisciplinary care team
- Coordinate care with therapists, social workers, home health aides, and other healthcare professionals
- Complete timely and accurate clinical documentation
- Educate patients and caregivers to promote health, safety, and independence
Qualifications
- Active Virginia Registered Nurse (RN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Critical Thinking Skills
- Excellent Communication and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS experience is a plus
