The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. We provide services and support to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the role
As a Housing Case Manager, you will support individuals and families who are experiencing homelessness or are at risk of homelessness by providing assessment, case management, counselling, advocacy, and referral services. Working within The Salvation Army's Specialist Homelessness Services team, you will deliver holistic, client-centred support, including assistance through the Private Rental Assistance Program to help prevent and end housing crises. This role requires strong collaboration with internal and external stakeholders to improve housing outcomes and support vulnerable people across the Wimmera Southwest region.

Reporting to the Wimmera Team Leader, this is a permanent part-time position, 30.4 hours per week, based in Horsham, VIC.
 

Conditions
Compensation is in accordance with the SCHADS Award – Level 5.
 

How you will make an impact

• Conduct comprehensive client assessments and provide tailored case management, advocacy, and support to individuals and families experiencing or at risk of homelessness
• Develop and implement holistic, strengths-based case plans and sustainable housing solutions that empower clients to achieve greater independence
• Support clients to access, secure, and maintain appropriate housing, including assisting households to establish successful private rental tenancies
• Coordinate referrals and connect clients with community services, employment pathways, training opportunities, and other supports to enhance wellbeing and self-sufficiency
• Maintain accurate client records, case notes, and reporting requirements while contributing to a collaborative team environment and delivering high-quality client outcomes
   

What you will bring

• Relevant tertiary qualification in Social Work or a related human services field, or equivalent demonstrated experience in case management and client support
• Demonstrated knowledge of homelessness including context, causes, issues, philosophies and legislation
• Ability to apply homelessness models, approaches, theories and practices when dealing with clients
• Ability to understand and respond to the needs of individuals experiencing homelessness or at risk of homelessness
• A current VIC Driver’s Licence
• A current Employee Working with Children Check
