Care Coordinator and Group Facilitator (Bilingual Spanish)

Position Details 

Title: EMPOWR Intensive Care Coordinator (ICC), Bilingual Spanish

Classification: Regular Full Time (1.0 FTE), Non-exempt (Hourly)

Location: Oakland, CA / Hybrid (availability to provide client services in-person and complete documentation remotely)

Regular Work Schedule: Full time, M-F 

Compensation:

• $38.08 per hour for bilingual Spanish candidates, inclusive of 10% language differential

The Team-Based model of care program named EMPOWR (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

The Intensive Care Coordinator (ICC) provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. This position is responsible for engaging and assisting foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family’s identified goals. 

The position draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. Because of their lived experience, they help create a connection with family and other close contacts, and offer a unique perspective to the rest of the team. 

How you'll make an impact:

• Outreach to and engagement with youth; being a strong advocate for the youth within the team
• Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living. 
• Provide intensive skill building with youth, with a focus on independent living.
• Provide assessments, crisis intervention, stabilization, and safety plans. 
• Identify and connect youth & young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities. 
• Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.
• Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services.
• Provide advocacy, targeted case management and community based rehabilitation services to children and their families. 
• Provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.)
• Provide stabilization support services to children and TAY in an in-person milieu environment.

Qualifications:

• Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.
• Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)
• Bilingual in Spanish (required)*
• Experience providing case management and rehabilitation services.
• At least two (2) years experience working with system-involved youth.
• Demonstrated ability to work with families who have experienced high levels of trauma and demonstrate high mental health needs.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Proven ability to be organized, manage time well, complete high quality and timely documentation.
• Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office. Clean CA DMV record, current car insurance and a vehicle; travel is required.

Competencies (skills, abilities, and knowledge):

• Demonstrated experience working with children and families who may be involved in the child welfare system.
• Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
• Cultural humility - knowledge of issues of race, class and ethnicity.
• Efficiency and time management - ability to be well-organized and complete timely documentation.
• Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
• Ability to work with clients who have complex traumatic histories and other attachment difficulties.

*Spanish Language Proficiency Requirement

This is a bilingual Spanish/English position. Candidates must demonstrate advanced Spanish language proficiency in order to effectively provide services to Spanish-speaking clients and families.

Final candidates will be required to complete an ALTA Language Proficiency Test administered by WestCoast Children’s Clinic and must achieve a minimum score of Level 9 (Advanced) to qualify for the position.

A Level 9 proficiency indicates the ability to:

• Participate in in-depth conversations in Spanish across a broad range of topics at a normal rate of speech
• Understand most spoken Spanish in professional and conversational contexts, using context to interpret unfamiliar slang, idioms, or complex grammar
• Communicate clearly and effectively on a wide range of subjects, even if some grammatical errors or a noticeable accent are present
• Be readily understood by native Spanish speakers without causing misunderstanding
• WestCoast Children’s Clinic will provide the ALTA language assessment to eligible candidates during the hiring process.
