Medication Adherence & Access Team - Job Overview

Schedule: Monday-Friday (8AM-5PM) | Hybrid

Your role is crucial in ensuring that patients have seamless access to their medications, empowering them to lead healthier and fulfilling lives.

Key Responsibilities

  • Act as a liaison between Legacy clients and providers/pharmacies to minimize medication wait times.
  • Respond promptly to incoming prescription renewal requests within 48 hours following protocol.
  • Identify requests outside prescription renewal guidelines and seek assistance when needed.
  • Support the provider's plan for patient follow-up and continuity of care.
  • Proactively identify and address patients at risk of losing prescription access.
  • Monitor and respond to incoming prior authorization requests efficiently.
  • Submit prior authorizations on behalf of providers and patients, ensuring accurate documentation and updates.
  • Maintain precise documentation within Legacy's EMR and Pharmacy system.
  • Contribute to the success of the team by performing additional duties as assigned.

Minimum Qualifications

  • High School Diploma or equivalent required.
  • Certification as a Pharmacy Technician, or LVN.
  • Minimum 5 years experience as a certified pharmacy technician, or LVN.
  • Demonstrates written and verbal communication skills.
  • Demonstrates excellent customer service skills.
  • Understanding of medication prescriptive process.
  • Computer software knowledge.
  • Proficiency in navigating electronic medical records.
  • Spanish speaking preferred.
