PRN Home Health Registered Nurse (RN)
Burlington to Andover
Are you an experienced Home Health RN looking for a part-time position that offers flexibility, autonomy, and meaningful one-on-one patient care?
Elara Caring is seeking a compassionate PRN Registered Nurse to provide skilled nursing care to patients in their homes. This role is ideal for an RN who understands home health operations, can work independently, and values building meaningful relationships with patients and families. PRN scheduling is available, but dependable and consistent availability is required (whether that is 4 hours/week or 12 hours/week).

What You’ll Do
Complete admissions, assessments, routine visits, recertifications, and discharges
Develop and update individualized plans of care
Monitor patients’ conditions and communicate significant changes
Coordinate care with physicians, caregivers, and the interdisciplinary team
Educate patients and families about medications, conditions, safety, and self-care
Complete accurate and timely clinical documentation
Help prevent avoidable hospitalizations through proactive nursing care
What You’ll Need
Current, unrestricted Massachusetts RN license
Previous home health nursing experience required
Ability to work independently and manage visits efficiently
Availability to commit to a consistent part-time schedule
Valid driver’s license, reliable transportation, and current auto insurance
Willingness to travel throughout Bellingham and surrounding communities
Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
