Description
The Portage County Health & Human Services department is hiring a Case Manager to support the Portage House. This is a full-time (40 hrs / week) Monday - Friday position.
Pay range starts at $23.32 / hr
This position provides morning, afternoon and evening coverage at the Portage House, maintains order and safety during shift, provides case management supervises and interacts with clients and ensures the house is kept clean and all rules, policies, and procedures are followed.
What are the primary job duties?
Client Case Management and Supervision
- Provide Case Management for identified clients.
- Conduct and keeps notes on weekly individual counseling sessions with each assigned client.
- Facilitate selected educational/training modules.
- Facilitate house meetings.
- Attend weekly staff meetings.
- Supervise and interact with clients (listen to concerns, encourage pro-social behavior, confront unsafe behavior.)
- Respond to rule or policy infractions by writing disciplinary reports. Formulate, assign and record disciplinary actions for identified clients.
- Record client clients’ activity and document and complete client reports as assigned.
- Provide case management and case planning with Department of Community Corrections (DCC) agents and other service providers.
- Attend supervisory sessions as scheduled.
- Attend/conduct client interviews.
- Perform intake and departure procedures with clients
House Coverage and Supervision
- Maintain order and safety during shift.
- Monitor clients’ activities and whereabouts.
- Ensure house rules, policies, and clients’ commitments are being followed.
- Answer phone, takes messages, and communicates to other staff regarding clients’ activities and behavior.
- Remain in-house during shift when working independently.
- Monitor visitors.
- Provide first aid to clients as needed.
- Provide limited training to Relief and Night Security staff.
- Assist with scheduling.
- Supervise Night Security and Relief staff and student interns.
House Management
- Responsible for house cleanliness during shift.
- Assign and monitor clients’ house chores.
- Monitor clients’ activities during emergencies to ensure safety and state code compliance.
- Assign and approve menus.
- Monitor house maintenance and repair needs.
What are the minimum qualifications?
- High School diploma or equivalent required. A minimum of two years’ post-secondary education in psychology, sociology, social work, or related human services field, or comparable training and experience required.
- Group and individual case management experience.
- Any combination of education and experience that provides the equivalent knowledge, skills and abilities required to perform the essential functions of the position.
- This position requires travel for work-related purposes in certain situations with residents. Must possess a valid driver's license and maintain personal automobile insurance coverage when using personal vehicle for business purposes.
- Ability to successfully pass a criminal background check.
