Position Summary
The Medical Assistant provides clinical and administrative support to Graceful Aging Home Health Care’s clinical
leadership and field staff. This position assists with coordination of patient care, medical records, physician orders,
medication documentation, scheduling, communication with healthcare providers, and other activities necessary to
support efficient and compliant home health operations.
The Medical Assistant works under the direction and supervision of the Director of Nursing/Clinical Manager and
agency leadership. The Medical Assistant does not independently perform nursing assessments, develop plans of care,
provide skilled nursing services, or perform duties outside of his or her education, training, certification, or permitted
scope of practice.
Reporting Relationship
Reports to the Director of Nursing/Clinical Manager and/or Director of Home Health Operations.
 
Essential Responsibilities
 
Clinical Administrative Support
  •  Provide administrative and clinical support to the Director of Nursing, Clinical Manager, nurses, therapists, and other members of the interdisciplinary team.
  •  Assist with organizing and maintaining patient clinical records.
  •  Review patient charts for completeness and identify missing documents, signatures, orders, or other required
  • information for follow-up.
  •  Assist with tracking physician orders, plans of care, certifications, recertifications, discharge documents, and other
  • clinical paperwork.
  •  Fax, securely transmit, and follow up on orders and other documents sent to physicians and healthcare providers.
  •  Maintain documentation of transmission and receipt of clinical documents as required by agency policy.
  •  Contact physician offices and other healthcare providers to follow up on outstanding orders, signatures, medical
  • records, and other documentation.
  •  Upload and appropriately file received medical records and clinical documents into the electronic medical record.
  •  Assist clinical leadership with maintaining tracking logs and compliance reports.
  •  Support preparation of patient charts for clinical review, audits, surveys, and quality assurance activities.

 Patient Care Coordination

  •  Assist with communication between patients, families, physicians, hospitals, pharmacies, medical equipment
  • providers, and agency clinical staff.
  •  Receive patient or caregiver calls and appropriately route clinical questions or concerns to a licensed nurse.
  •  Assist with scheduling and coordinating home health visits as directed.
  •  Help coordinate appointments, referrals, laboratory services, diagnostic testing, medical equipment, and other
  • services ordered for patients.
  •  Follow up with patients and caregivers regarding non-clinical scheduling and coordination matters.
  •  Promptly communicate reports of changes in patient condition, medication concerns, falls, hospitalizations,
  • emergency room visits, or other clinical concerns to the appropriate licensed clinician.
  •  Assist with coordinating resumption of care following hospitalization or other transitions in care.
  •  Support discharge and transfer coordination as directed by clinical leadership.
Referral and Admission Support
  • Assist with receiving and processing new patient referrals.
  • Obtain required referral documentation, including demographics, insurance information, physician information,
  • hospital records, medication lists, and supporting clinical documentation.
  • Assist in verifying that required referral documentation has been received before admission.
  •  Communicate missing referral information to referral sources and follow up until requested records are received.
  • Enter referral and patient information into the agency's electronic medical record.
  •  Assist with coordinating initial patient visits with appropriate clinical personnel.
  • Maintain referral and admission tracking information.
  • Communicate potential clinical concerns identified during the referral process to the Director of Nursing or other
  • licensed clinician for review.
Physician Orders and Documentation
  •  Maintain systems for tracking outstanding physician orders and plans of care.
  •  Submit physician orders and other documents for signature in a timely manner. Follow up with physician offices regarding unsigned or outstanding documents.
  •  Document follow-up efforts according to agency procedures.
  •  Ensure signed orders are uploaded and filed in the appropriate patient's clinical record.
  •  Notify clinical leadership of orders that remain outstanding or require escalation.
  •  Assist with monitoring documentation deadlines and alert appropriate staff when documentation is approaching or
  • exceeding established timelines.
Medication Documentation Support
  •  Assist with obtaining medication lists from patients, hospitals, physician offices, pharmacies, and other authorized healthcare providers.
  •  Enter or update medication information in the electronic medical record when directed and reviewed by a licensed
  • clinician.
  • Identify apparent discrepancies between medication lists and refer them to a Registered Nurse or other appropriate
  • licensed clinician for reconciliation.
  •  Assist nurses with obtaining clarification from physician offices or pharmacies regarding medication documentation.
  •  The Medical Assistant may not independently perform medication reconciliation, make medication-related clinical
  • decisions, or provide medication instructions to patients.
Quality Assurance and Compliance Support
  •  Assist the Director of Nursing and agency leadership with routine chart audits and compliance monitoring.
  •  Review records for administrative completeness using agency-approved checklists and audit tools.
  •  Track missing documentation and communicate deficiencies to the appropriate staff member.
  •  Assist with maintaining logs related to admissions, orders, hospitalizations, transfers, discharges, incidents,
  • complaints, and other agency quality indicators.
  •  Assist with preparation for state, federal, Medicare, accreditation, and other regulatory surveys.
  •  Participate in quality improvement activities as assigned.
  •  Maintain patient confidentiality and comply with HIPAA requirements.
  •  Follow all Graceful Aging Home Health Care policies and procedures.
  • Report compliance, documentation, patient safety, or operational concerns to appropriate agency leadership.
Office and Administrative Responsibilities
  •  Answer and appropriately route telephone calls.
  •  Communicate professionally with patients, caregivers, physicians, referral sources, vendors, and other healthcare
  • professionals.
  •  Assist with scanning, filing, faxing, copying, and maintaining clinical records.
  •  Monitor incoming faxes and clinical communications and route documents to the appropriate staff member.
  •  Assist with maintaining organized electronic and physical records.
  •  Support agency leadership with reports, spreadsheets, tracking tools, and special projects.
  •  Attend staff meetings, training sessions, and in-service education as required.
  •  Perform other related duties as assigned.
Required Qualifications
  •  High school diploma or equivalent.
  •  Completion of a Medical Assistant training program.
  •  Medical Assistant certification or registration preferred.
  •  Minimum of one (1) year of experience in a medical office, physician practice, hospital, home health agency, or other
  • healthcare setting preferred.
  • Current CPR certification preferred.
  •  Successful completion of required criminal background checks and health screenings.
Highly Preferred Qualifications
  •  Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), Certified Clinical Medical Assistant (CCMA), or equivalent credential.
  •  Previous experience working in a home health agency.
  •  Experience working with Medicare-certified healthcare providers.
  •  Experience processing physician orders and plans of care.
  •  Experience communicating with physician offices, hospitals, pharmacies, and other healthcare providers.
  •  Experience conducting medical record or documentation audits.
  •  Familiarity with home health admissions, recertifications, transfers, resumptions of care, and discharges.
  • Experience using Axxess or another home health electronic medical record system.
  •  Experience with Microsoft Office, spreadsheets, electronic fax systems, and document management.
Employment Requirements
  •  Successful criminal background check.
  •  Required health screening and TB testing.
  •  Drug screening as required by agency policy.
  •  Ability to maintain patient confidentiality.
  •  Reliable transportation if travel is required.
  •  Compliance with all agency training and competency requirements.
