Health Advocacy to Support Quality Care: You Can Make A Difference   

Bring your talents to NewYork-Presbyterian and help drive the success of our Center for Community Health Navigation’s Health Promotion Program.  

The Health Promotion Advocate (HPA) supports NewYork-Presbyterian's emphasis on health equity by screening patient for social needs and, when needs are identified, connecting patients to resources to address their needs.  HPAs also empower patients to understand how to enroll onto and to navigate our patient portal so that they can access their records, schedule appointments, and communicate with providers.

 

Health Promotion Advocate - Health Promotions Program – Days 

 

The Health Promotion Advocate administers social determinant of health screening, connects patients to resources, and empowers patients to navigate the patient portal. Advocates are responsible for carrying out the duties for the assigned program and site. 

 

 

Preferred Criteria 

• Community-based and/or healthcare experience
• Bachelor's degree 
• Fluent in Spanish

 

Required Criteria 

 

• High School diploma or GED 
• Strong written and verbal communication skills
• Microsoft Word and Excel experience

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
