POSITION SUMMARY:
- The RN is experienced in planning, organizing, and directing home health services with an emphasis on holistic care.
- The professional RN builds from resources to plan and direct services to meet the needs of individuals and their family members within their homes and communities.
- Completes an initial assessment of the patient and family to determine home care needs, including a complete physical assessment and history of current and previous illnesses.
- Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis, incorporating therapeutic, preventative, and rehabilitative nursing actions.
- Provides direct patient care as defined in the State Nurse Practice Act.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
- Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNAs, and MSWs.
QUALIFICATIONS:
- An associate's or Bachelor’s degree with one year of home health care experience preferred.
- AZ Nursing licensure in good standing.
- Current CPR/BLS certification.
- Fingerprint clearance card.
- TB test results within the last year.
- AZ Driver's license and proof of insurance with reliable transportation.
- Excellent observation, verbal and written communication, and problem-solving skills.
