POSITION TITLE: Residential Case Manager 

                   

PROGRAM/LOCATION: Community Residential Programs / San Luis Obispo   


POSITION SUMMARY:

The Community Residential Program (CRP) provides long-term, affordable, semi-independent housing with supportive case management for adults living with severe mental illness who are actively engaged in treatment through San Luis Obispo County Behavioral Health. CRP promotes recovery, autonomy, and community integration by offering a stable home environment with individualized support that empowers residents to reach their highest level of independence. Case Managers for the Community Residential Program are expected to support residents in maintaining long-term, stable housing while encouraging independent living skills and self-sufficiency. Case Managers for the Community Residential Program are expected to supervise the general health, safety, and social rehabilitation of clients. They also perform moderate facility maintenance, function as positive role models, promote cooperation and harmony in communal living, and uphold and promote the program philosophy in all duties performed. This position is full time, 40 hours a week, including weekends, the regular weekly work schedule for this position is Wednesday through Sunday. 


HOURS/ SALARY EMPLOYEE PERKS:

Full-time, non-exempt position. Mandatory work days Wednesday through Sunday. 40 hours per week. Anticipated hiring range is $25.50-$26.50. Includes a generous individual benefits package that includes health, dental, vision, life, 403b retirement (with up to a 5% employer match) paid vacation/holiday time, paid sick time and a variety of helpful employee perks as well. (*Full Salary Schedule $25.50-$33.99 per hour).

MISSION STATEMENT:

Transitions Mental Health Association is a nonprofit organization dedicated to eliminating stigma and promoting recovery and wellness for people with mental illness through work, housing, community and family support services.  


RESPONSIBILITIES AND DUTIES:

1. Assist residents in all aspects of independent living including the development of social, vocational, and effective life management skills; and assisting residents with proper nutrition, cooking, shopping, budgeting, housecleaning and maintenance, and personal hygiene.

2. Recognize and be available to clients in psychiatric crisis or emotional distress and teach appropriate coping and conflict resolution skills.  Provide counseling and emotional support.

3. Facilitate recreational, educational, or other program activities.  This may include transporting clients, encouraging client participation, and modeling appropriate social behavior.

4. Facilitate client and house meetings weekly. House meetings provide opportunities for education, discussion and mediation between housemates. Client meetings provide opportunities to discuss individual clients’ needs and goals. 

5. Assume the role of advocate in all aspects of clients’ needs and goals.  This may include familiarizing clients with and encouraging the usage of community resources, support systems, services, and opportunities; and assisting clients in building healthy peer and familial support networks. 

6. Promote, encourage and maintain trauma-informed practices in all services and activities. 

7. Practices and promotes, both individually and in team meetings, the Collaborative Communication Guidelines.

8. Assist clients in determining, developing, and carrying out their own goals; review plans and goals, evaluate progress, and foster independence.

9. Maintain the cleanliness and upkeep of the residences; this may include general household chores, yard work, completion and/or reporting of repairs needed, acquisition and disposal of furniture, and assisting clients in maintaining a clean and sanitary living environment. Complete monthly facility checks. 

10. Complete program paperwork: this may include individual client files, admission/ discharge paperwork, petty cash receipts and vehicle checklists. 

11. Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Program Manager and Director.

12. All direct service staff are expected to spend at least 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.

13. Act as agency liaison by consulting, collaborating, and communicating with Program Manager, SLO County Behavioral Health, and other treatment agencies to meet client and program objectives. This may include coordinating referrals and/or providing information to other agencies or treatment programs.

14. Attend weekly staff meetings and complete required hours of in-service training.

15. Assume other duties and responsibilities as assigned by the Program Manager.


JOB ENVIRONMENT:

1. Moderate, physical activity and frequent driving; may include moving or lifting up to 50 lbs.

2. Involves moderate risks that require safety precautions and the risk of exposure to potentially dangerous situations.  


Requirements

Minimum Requirements:

1. BA/BS AND 6 months of full-time experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time experience with target population, OR 4 years of full-time experience with target population.  

2. Must be available to work the regular Wednesday through Sunday schedule that includes the weekend.

3. Strong interpersonal skills, collaborative work style and strong oral/written and communication skills.  

4. Well defined organizational and time management skills. 

5. Ability to work independently and flexibly.

6. Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure. 

7. Able to work effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level.

8. Ability to possess and maintain good physical and mental health.  

9. Ability to work well within a team and maintain a professional, respectful attitude in the workplace.

10. Ability to be willing to work with supervisor oversight and direction. 

11. Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees.

16. Conditions of employment include: 

• Must be at least 18 years of age; possess a valid CA Driver’s License, at least 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements.  May be required to utilize personal vehicle and auto insurance.  

• Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting. 

• Must be able to successfully complete First Aid and CPR training.
