About the Role
We are seeking a compassionate Primary Care Nurse Practitioner (NP) to provide high-quality medical care to veterans in Oswego, NY. In this role, you will collaborate with an integrated primary care team (PACT) to deliver comprehensive outpatient management and drive superior clinical outcomes.
Key Responsibilities
Examine, diagnose, and manage primary care patients in an outpatient setting.
Develop, implement, and maintain continuous patient management plans.
Order, perform, and interpret diagnostic tests (X-rays, ECGs, laboratory tests).
Prescribe medications and advise patients on treatment, nutrition, and mental health maintenance.
Complete all clinical documentation accurately within 24 hours of patient encounters.
Ensure strict adherence to government/federal healthcare guidelines, formulary protocols, and compliance standards.
Requirements & Qualifications
Education: Master’s degree in Nursing (MSN) or higher from an accredited Nurse Practitioner program.
Experience: Combined academic training and primary care/ambulatory care NP experience (government/veteran healthcare setting preferred).
Licensure & Certification:
Active, unrestricted APRN / Nurse Practitioner license.
National Board Certification in Family, Adult, or Gerontological Primary Care.
Credentialed and in good standing through federal/veteran healthcare guidelines.
Active, unrestricted DEA registration.
Current BLS (and ACLS, if applicable) certification.
