Shift: Days | 8-Hour Shifts.
Hours: 40 Hours Guaranteed.
Weekend Requirement: None.
On-Call: None.
Mileage Reimbursement: $0.43 per mile.
Job Summary:
We are seeking an experienced Physical Therapist (PT) for a temporary/travel Home Health assignment in Kāneʻohe, Hawaii. In this role, you will provide high-quality, patient-centered physical therapy services in patients' homes, helping them restore mobility, improve function, and achieve their rehabilitation goals.
Working collaboratively with an interdisciplinary healthcare team, you will evaluate patients, develop individualized treatment plans, deliver evidence-based therapy, and coordinate discharge planning while maintaining accurate clinical documentation.
Responsibilities:
- Perform comprehensive physical therapy evaluations in the home health setting.
- Develop individualized treatment plans based on patient needs and goals.
- Deliver evidence-based physical therapy interventions.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals.
- Complete timely and accurate documentation in Epic EMR.
- Educate patients and caregivers on home exercise programs and safety strategies.
- Supervise Physical Therapist Assistants (PTAs) as needed.
- Participate in discharge planning and continuity of care.
Qualifications:
Required:
- Active Hawaii Physical Therapist License (must be active at the time of submission).
- Minimum of 2 years of Physical Therapist experience.
- At least 1 year of Home Health Physical Therapy experience.
- Experience using Epic EMR.
- Bachelor's Degree in Physical Therapy (Master's preferred).
- Current BLS (AHA) and CPR certification.
Preferred:
- Previous travel Physical Therapist experience.
