Description
The Home Care PT evaluates the patient functional needs, creates customized treatment plans, and delivers one-on-one therapy to help individuals recover from surgery, injury, or chronic illness while living at home.
Responsibilities
- Patient Evaluations: Conduct initial assessments, start-of-care visits, and regular re-evaluations to measure functional independence and progress
- Custom Care Plans: Design and update personalized treatment plans under a physician's direction.
- Therapeutic Interventions: Provide gait and balance training, muscle strengthening, pain management, and instruction on using assistive devices (walkers,canes).
- Home Safety Assessments: Inspect the patient's living environment for safety hazards and recommend necessary modifications or accessibility tools.
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Minimum of 1 year of clinical or home health care experience preferred
- Valid driver's license, reliable personal automobile, active auto insurance, and willingness to drive to patients homes
- Effective written and verbal communication
- Ability to stand, bend, lift and transfer patients (frequently lifting up to 50-100 lbs)
- US Citizen or Green Card holder
- Current, active, and unrestricted Physical Therapist state license
