About the Role

The Care Coordinator / Senior Occupational Therapist provides care coordination and discipline-specific clinical services to ALERT clients, while providing clinical leadership within Complex Care Services (CCS) across both Occupational Therapy and care coordination functions. As the Occupational Therapy Senior Clinician, you will provide leadership and support to Occupational Therapy staff within ALERT/CCS, liaise with the St Vincent’s Hospital Melbourne Occupational Therapy Department, and contribute to professional development activities.

Key leadership responsibilities include:

• Reviewing clinical documentation and identifying opportunities for improvement

• Supporting service improvement and quality initiatives

• Providing peer support and individual supervision

• Facilitating professional development opportunities

• Promoting high-quality, evidence-based Occupational Therapy practice

Classification: Occupational Therapist Grade 3 ($104,135 - $118,981 per annum, pro rata for part time).

As a Care Coordinator, you will provide comprehensive and holistic health assessments and develop person-centred, client-driven goals focused on care coordination and improved self-management.

The role involves both direct and indirect clinical care, maintaining an active caseload, and supporting clients to develop self-management skills in relation to their complex care needs and/or chronic disease.

Your Contribution

To be successful in this role, you will have:

• A recognised qualification in Occupational Therapy

• Current AHPRA registration

• Eligibility for membership with Occupational Therapy Australia

• A minimum of 7 years' experience in a relevant acute and/or community setting

• Demonstrated clinical expertise and knowledge of best-practice Occupational Therapy interventions

• Experience in care coordination for clients with chronic and complex health needs

• Strong communication, collaboration and leadership skills

• A commitment to person-centred, trauma-informed and multidisciplinary care

About ALERT

This position is part of the ALERT Team within the Health Care for Homeless Program (HfH). The HfH Program provides a range of services within St Vincent’s to support people who are experiencing, or are at risk of, homelessness. The program aims to strengthen the integration and coordination of existing services across St Vincent’s Hospital and develop new services to respond to the needs of the target population. Operationally, the role has strong links with the Health Independence Program (HIP). HIP provides a comprehensive range of specialist clinics, services and rehabilitation programs, including rapid-response health management, short- to medium-term care coordination and health education. HIP supports clients to be active decision-makers in their care and assists them to develop self-management skills to achieve their healthcare goals.

ALERT supports people with complex needs who experience one or more key risk factors, including:

• Substance use

• Mental health concerns

• Homelessness

• Disability

• Complex aged care needs

• Family violence

The position is based at St Vincent’s Hospital Melbourne, primarily within the Emergency Department, and also provides support and interventions in the community as part of an interdisciplinary team.
