The Emergency Room RN Case Manager, in collaboration with internal and external interdisciplinary teams, coordinates and directs patient care throughout the emergency department stay and transition process. The role promotes safe, timely, and cost-effective care through comprehensive assessment, discharge planning, resource management, utilization review, patient advocacy, and collaboration with patients, families, providers, and community partners. This position supports optimal patient outcomes while ensuring appropriate utilization of healthcare resources and exceptional customer service.
Responsibilities   
  • Key Responsibilities of an ED RN Case Manager

    Patient Assessment

    • Review patients for medical necessity and level of care.
    • Assess psychosocial, financial, and discharge planning needs.
    • Identify barriers to care, including transportation, housing, insurance, medication access, and caregiver support.

    Care Coordination

    • Collaborate with ED physicians, nurses, social workers, hospitalists, and specialists.
    • Coordinate admissions, transfers, and observation status placements.
    • Facilitate communication between patients, families, and the healthcare team.

    Utilization Management

    • Review admission criteria and medical necessity, working with our UR dept

    Discharge Planning

    • Develop discharge plans early in the ED stay.
    • Arrange home health services, durable medical equipment, rehabilitation, skilled nursing facility placements, or hospice services when appropriate.
    • Coordinate follow-up appointments and community resources.

    Patient and Family Education

    • Educate patients and families regarding treatment plans and discharge instructions.
    • Explain care options, insurance considerations, and available resources.
    • Promote self-management and prevent avoidable return visits.

    Community Resource Management

    • Connect patients with:
      • Behavioral health services
      • Substance use treatment programs
      • Housing and shelter resources
      • Transportation assistance
      • Financial and medication assistance programs

    Throughput and Length-of-Stay Management

    • Assist with reducing ED boarding and delays in care.
    • Expedite placement decisions and discharge arrangements.
    • Support patient flow initiatives to improve ED efficiency.

    High-Risk Patient Management

    • Identify frequent ED utilizers and high-risk populations.
    • Develop care coordination plans to reduce avoidable readmissions and repeat ED visits.
    • Collaborate with outpatient care managers and population health teams.

    Common Performance Metrics

    • Discharge disposition outcomes
    • Patient satisfaction

Requirements/Qualifications

  • Associates degree from an accredited RN program
  • Current license to practice as registered nurse in the State of Ohio
  • Minimum of 3-5 years recent clinical experience in an acute care setting.
