Position Summary
The Intake Specialist serves as an essential first point of contact for patients accessing care at Bond Community Health Center. Under the direction of the appropriate supervisor, the Intake Specialist is responsible for completing patient intake and registration processes, verifying demographic and insurance information, assisting patients with required forms and documentation, and ensuring accurate information is entered into the electronic health record.
This position provides professional, courteous, and culturally responsive customer service while maintaining patient confidentiality and compliance with applicable healthcare regulations, organizational policies, and procedures.
Requirements
Essential Duties and Responsibilities
- Greet and assist patients, visitors, and others in a professional, courteous, and welcoming manner.
- Complete patient registration and intake processes accurately and efficiently.
- Obtain and verify patient demographic, contact, insurance, and eligibility information.
- Assist new and established patients with completion of registration, consent, financial, and other required forms.
- Review patient information for completeness and accuracy and make appropriate updates in the electronic health record.
- Verify insurance coverage, eligibility, and other required payer information as applicable.
- Explain registration, financial, and administrative requirements to patients in a clear and respectful manner.
- Identify patients who may qualify for available programs, sliding-fee assistance, insurance coverage, or other resources and make appropriate referrals.
- Collect required copayments, fees, or other patient financial obligations in accordance with established procedures.
- Coordinate with clinical, billing, referrals, outreach, and other departments to facilitate timely access to services.
- Respond to patient questions and concerns and escalate issues appropriately when additional assistance is needed.
- Maintain accurate and confidential patient records in accordance with HIPAA and organizational requirements.
- Ensure required documentation is obtained and appropriately scanned, uploaded, or filed.
- Assist with scheduling and appointment-related activities as assigned.
- Support patient flow by communicating relevant information to appropriate departments and staff.
- Follow established procedures for handling sensitive, confidential, and protected health information.
- Maintain knowledge of Bond Community Health Center services, programs, policies, and community resources.
- Participate in staff meetings, training programs, and professional development activities.
- Demonstrate a commitment to Bond Community Health Center's mission, values, and commitment to providing accessible, quality healthcare.
- Perform other duties as assigned.
Required Qualifications
- High school diploma or GED required.
- One (1) year of experience in healthcare, medical office, patient registration, customer service, or a related administrative environment preferred.
- Experience working with electronic health records and/or healthcare information systems preferred.
- Basic knowledge of medical terminology preferred.
- Strong verbal and written communication skills.
- Strong organizational and attention-to-detail skills.
- Ability to communicate effectively and professionally with patients from diverse backgrounds.
- Ability to maintain confidentiality and handle sensitive information appropriately.
- Ability to work effectively in a fast-paced healthcare environment.
- Basic computer proficiency, including Microsoft Office and electronic data-entry systems.
Preferred Qualifications
- Previous experience in a Federally Qualified Health Center (FQHC), community health center, primary care, or other outpatient healthcare setting.
- Experience with patient registration, insurance verification, eligibility screening, or financial assistance programs.
- Bilingual or multilingual skills are a plus.

Physical Requirements
This position may require prolonged periods of sitting or standing, frequent computer and telephone use, and regular interaction with patients and staff. The employee must be able to perform the essential functions of the position with or without reasonable accommodation.
