Patient Care Coordinator- BCBS Mississippi

Overview

Together Women’s Health is seeking a remote Patient Care Coordinator (PCC) to support our Mississippi practices on Blue Cross Blue Shield of Mississippi (BCBS MS) quality initiatives, including Healthy You! annual wellness exams and Color Me Healthy! (CMH!) enrollment. The PCC confirms patient eligibility before each visit, supports provider documentation workflows, and follows up on CMH! activations so patients receive the full benefit of these programs.

 

Experience with athena and the BCBS MS provider portal is highly encouraged.

Responsibilities

· Review upcoming annual appointments on a daily basis and identify whether each patient is eligible for BCBS MS benefits using athena and the BCBS MS portal.

· Prepare eligible patient’s chart to notify the provider of the patient’s eligibility for Healthy You! and/or CMH!, including updating the custom fields, adding the eligibility sticky note, and teeing up appropriate labs.

· Review daily BCBS CMH! activation response files, call newly activated patients to schedule CMH! Follow-up throughout the year, and schedule patients in the practice’s athena schedule

· Communicate clearly and courteously with patients by phone and secure messaging, and document all outreach in athena.

Qualifications

· High school diploma or equivalent.

· Experience with athena is highly preferred.

· Experience with the BCBS provider portal is highly preferred.

· Excellent interpersonal and telephone communication skills, with a courteous and friendly manner at all times.

· Detail-oriented and highly organized, with the ability to manage a high volume of eligibility checks and scheduling tasks.

· Reliable high-speed internet and a private, secure workspace for handling protected health information (PHI) in a remote setting.
