Description

PACS Group, Inc. is a holding company investing in post-acute healthcare facilities, professionals, and ancillary services. Founded in 2013, PACS Group is one of the largest post-acute platforms in the United States. Its independent subsidiaries operate close to 400 post-acute care facilities with over 50,000 employees across 22 states. PACS business support division, PACS Services, provides technology and administrative support services — accounting, finance, human resources, compliance, payroll, AR/AP, legal, risk management, information technology, corporate communication, and other business advice and support — to their healthcare facilities, reducing administrative burdens so their leadership and care teams can focus on the care, well-being, and quality of life of their patients and residents. PACS is one of Utah's Best Companies to work for. It has also been recognized as one of Utah's Fastest Growing Companies for multiple years.

Job Summary/General Purpose

Serves as the primary liaison between PACS hospice providers and their contracted partner facilities, including skilled nursing, assisted living, and other long-term care settings. Manages hospice-facility service agreements, strengthens referral and care-coordination relationships, and helps ensure contractual and regulatory compliance. Works closely with hospice leadership, facility administrators, and PACS Services teams to support timely access to hospice care and sustainable census growth.

Key Responsibilities

  • Serve as the main point of contact between hospice teams and partner facilities, building and maintaining strong working relationships with administrators, directors of nursing, social services, and admissions staff.
  • Coordinate the drafting, negotiation, execution, and renewal of hospice-facility service agreements in partnership with Legal and Compliance, and maintain an accurate contract tracker with key terms and renewal dates.
  • Monitor adherence to contract terms, including care coordination, communication expectations, room and board arrangements, and billing processes, and address gaps promptly.
  • Ensure agreements and facility interactions align with applicable federal and state hospice regulations, including CMS Conditions of Participation, and support audit readiness.
  • Resolve service, communication, and care-coordination issues between hospice and facility teams, escalating complex or high-risk matters to leadership as appropriate.
  • Educate facility staff on the hospice benefit, eligibility, and referral processes to support timely, appropriate admissions and a consistent patient and family experience.
  • Track and report on referral volume, census, and partner facility performance, identifying trends and opportunities to expand and strengthen hospice partnerships.
  • Support onboarding of new partner facilities and new hospice locations, including orientation, documentation, and implementation of agreed workflows.
  • Collaborate with PACS Services teams, including Legal, Compliance, Finance, and Revenue Cycle, to resolve contract, billing, and compliance questions.

Requirements

  • Must: 2-4 years of experience in hospice, home health, post-acute care, or healthcare contract or account management
  • Must: Bachelor's degree in Healthcare Administration, Business, or a related field, or equivalent relevant experience
  • Must: No license required; a clinical license (RN, LPN, or LSW) is a plus
  • Must: Ability to travel locally to partner facilities (up to 25%) with a valid driver's license and reliable transportation
  • Optional: Experience with hospice-facility agreements, skilled nursing or assisted living operations, or healthcare business development a plus
  • Optional: Strong verbal and written communication, relationship-building, negotiation, and organizational skills; working knowledge of hospice regulations and proficiency with Microsoft Office and EMR or CRM systems
