Director of Patient Care Services

SUMMARY

The Director of Patient Care Services is responsible for the strategic and operational management of clinical services, including planning, organizing, staffing, and directing program activities consistent with the hospice's policies and procedures and with all applicable state and federal regulations. This position provides direct oversight Team Managers and Triage Manager and is accountable for the agency's compliance with California's hospice emergency regulations (Title 22, California Code of Regulations, sections 74800 through 74908, CDPH-18-002E, effective June 22, 2026) and the Medicare Conditions of Participation for Hospice Care (42 CFR Part 418). This position participates as a member of the Senior Leadership Team.

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego County and south Riverside County with multiple hospice office locations.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m. On call rotation to include holidays. Alternate Work Week-9/80.

On Call: On-call rotation with holidays.

Salary: $145,000 - $158,000 Annually

Work Location: Escondido, CA

Travel: Travel is required for staff supervision, public relations and marketing. Attendance at seminars, conferences and workshops may be required.

JOB DUTIES / KNOWLEDGE / JOB PERFORMANCE

CORE OPERATIONAL DUTIES

1. Direct the delivery of clinical services across all assigned care teams and ensure compliance with agency policy and applicable regulations.
2. Identify the need for changes in program design, develop proposals, and make recommendations to the Senior Leadership Team as appropriate.
3. Lead program development initiatives for clinical services.
4. Work collaboratively with the Senior Leadership Team on cross-functional projects, coordinating them with overall agency plans.
5. Direct the implementation of, and monitor agency compliance with, state health department and federal government standards and regulations for licensure and certification, including Title 22 CCR and 42 CFR Part 418.
6. Work cooperatively with the Chief Operating Officer, Chief Medical Officer, and other leaders in developing policies and procedures that support high-quality, compliant care.
7. Participate in workforce planning, recruitment strategy, and career development planning for clinical staff.
8. Oversee clinical care teams and programs as assigned, including RNCM, LVN, HHA, MSW, and spiritual care staff as applicable.
9. Provide guidance in communication, coordination, and effective operations through the development and use of recording and reporting systems, manuals, and other resources.
10. Directs utilization review activities and ensure findings are incorporated into staffing and program decisions.
11. Develop and oversee the annual operating budgets for assigned departments.
12. Provide input on clinical information systems and monitor the processing of data relating to clinical services and regulatory reporting.
13. Participate in professional and community activities that further the hospice's mission.
14. Partner with the Director of Quality and Compliance to exceed publicly reported industry standard outcomes, sustain CAHPS composite scores above state and national benchmarks, and ensure staff adherence to organizational care model standards.

Requirements

QUALIFICATIONS

1.  Meet one of the following educational and work experience criteria, consistent with Title 22 CCR section 74852: (a) a registered nurse (RN) with a baccalaureate or higher degree in nursing or other health-related field and a minimum of two (2) years of full-time supervisory or managerial experience in a hospice or home health agency within the last five (5) years; or (b) a registered nurse (RN) with a minimum of four (4) years of full-time supervisory or managerial experience in a hospice or home health agency within the last five (5) years.

2.  Working knowledge of Title 22 California Code of Regulations, sections 74800-74908 (CDPH-18-002E), and the Medicare Conditions of Participation for Hospice Care (42 CFR Part 418).

3.  Certified Hospice and Palliative Nurse (CHPN) certification required or must be obtained within twelve (12) months of hire.

4.  Current CPR certification.

5.  Current negative TB test or equivalent per agency policy.

6.  Active, unrestricted California Registered Nurse license.

7.  Current California driver's license, proof of auto insurance, and reliable transportation.
