Join our Saint Luke's team as a Diabetes Educator Registered Nurse!
Monday - Friday, 8am - 430pm
Strong knowledge in Diabetes and medication management
MO and KS RN license required
CDCES required within 2 years of hire
The Diabetes Educator Registered Nurse serves as a role model consultant change agent instructor and facilitator in assessing learning needs and in planning implementing and evaluating educational activities for nursing staff. Serves as a clinical and educational consultant to promote and support the professional practice environment maximum performance and quality patient care. Provides diabetes self management training for patients either newly diagnosed with diabetes or with identified diabetes needs such as hypoglycemia and diabetic ketoacidosis
The Opportunity:
Assessment of patients abilities, limitations and barriers to diabetes self-management
Collaboration with referring provider and patient care team to identify and prioritize strategies to assist patients with diabetes self-management education
Assist patients with medication adjustments to achieve optimal glycemic control in direct collaboration with the primary care provider
Understand the pathophysiology of diabetes, medication management, acute and chronic diabetes related complications and motivational interviewing
Use self-management training techniques to assess, plan and implement patient and family education in individual and/or group settings. Education to include self-monitoring of blood glucose, medications, diet/meal planning, exercise, goal setting, coping skills, and reduction of risk factors
Keep current in new and advancing diabetes related technologies
Participate in CQI projects and evaluation of outcome measures to assess diabetes education effectiveness.
