Job Summary:
The Home Health/Hospice RN will provide case management and skilled nursing services, supervise CNAs and PRN nurses, and coordinate care among home care disciplines, physicians, non-physician practitioners (NPP), and patients. They provide services that reflect the Mission, standards, and philosophy of the Hot Springs Health Program.
Job Relationships:
- Reports to: Madison Home Care & Hospice Director
- Persons supervised: CNA’s, LPN’s, and PRN Nurses
- Interrelationships: Interacts in person or on the phone with patients, family
- members, caregivers, physicians and medical center staff, discharge planners, other hospital personnel, all members of the health care team, and support staff.
Access to Protected Health Information:
The Home Health & Hospice RN must have access to clinical, billing, and contact information of patients as applicable to their job duties.
Essential Job Duties:
- Ensure quality and safe delivery of home care services, according to State, Federal, and local regulations as well as the NC Board of Nursing Practice Act.
- Perform technical skills and procedures as ordered by Providers.
- Effectively communicate with co-workers, patients, family, providers, and all ancillary staff.
- Supply ordering responsibilities.
- Accept responsibility for personal growth/Accountability/Time Management.
- Provide respite and/or continuous care for hospice patients.
- Attend all required meetings and trainings.
- Complete accurate and timely documentation.
- Assist with on-call duty when needed.
- Case management to include education to patients and families on proper home health care procedures and strategies.
- Provide supervision of CNA’s, LPNs, and PRN nurses in the home by verifying they are following the plan of care, infection prevention and control, and reporting any changes in the patient’s condition.
- Other duties as assigned.

Education/Experience:
- Education: Graduate of an accredited school of nursing and currently holding an RN license in the State of North Carolina.
- Experience: Minimum of one (1) year of experience preferred; home health or hospice experience preferred.
- Other: Valid N.C. Driver’s License, proof of auto liability insurance, and dependable transportation.
