Description:

The Transplant Coordinator RN plays a critical role in supporting patients throughout the heart transplant journey, serving as the primary clinical resource during both the pre-transplant and post-transplant phases. This role coordinates complex patient care activities, collaborates with a multidisciplinary transplant team, provides education and support to patients and families, and ensures compliance with regulatory and transplant governing organizations, including UNOS. The successful candidate will help deliver exceptional patient outcomes through expert clinical coordination, education, advocacy, and quality-focused care.

What You Will Do

  • Coordinate the clinical care of heart transplant patients throughout the pre-transplant and post-transplant continuum.
  • Facilitate comprehensive transplant evaluations by partnering with transplant physicians, surgeons, social workers, dietitians, insurance coordinators, and scheduling teams.
  • Review laboratory results, diagnostic testing, and clinical information to support patient evaluations and medical review board discussions.
  • Serve as a primary point of contact for transplant patients and families, providing guidance, support, and care coordination throughout the transplant process.
  • Educate patients and caregivers regarding transplant procedures, medications, follow-up care, nutrition, and long-term wellness management.
  • Conduct telephone triage, assess patient needs, and coordinate timely interventions with the healthcare team.
  • Maintain accurate patient records and documentation within the electronic medical record (EMR).
  • Obtain, review, and discuss consent forms, treatment plans, and potential complications with patients and families.
  • Ensure accurate transplant waitlist management by maintaining communication with the United Network for Organ Sharing (UNOS).
  • Prepare and submit required information to regulatory agencies and organizations including UNOS, CMS, Joint Commission, insurance providers, and organ exchange programs.
  • Participate in quality improvement initiatives, outcomes monitoring, and preparation of quality reports.
  • Collaborate with interdisciplinary teams to continuously improve patient care, transplant processes, patient safety, and service excellence.
  • Attend departmental meetings, educational programs, conferences, and committee activities as assigned.
  • Provide cross-functional support across transplant program operations, including pre-transplant, post-transplant, and procurement-related activities as needed.

What You Will Bring

Required Qualifications

  • Associate Degree in Nursing (ADN).
  • Current Illinois Registered Nurse (RN) License.
  • Current CPR/BLS Certification.
  • 1-2 years of relevant nursing experience.
  • Strong clinical assessment, care coordination, and patient education skills.
  • Excellent interpersonal, communication, and relationship-building abilities.
  • Ability to manage multiple priorities in a fast-paced, highly specialized clinical environment.
  • Strong organizational skills and attention to detail.

Preferred Qualifications

  • Bachelor's Degree in Nursing (BSN).
  • 3-5 years of professional nursing experience.
  • Previous experience in transplant services, dialysis, outpatient care, nephrology, cardiology, or other related specialty areas.
  • National Certification as a Transplant Coordinator or certification in a related nursing specialty.
  • Experience working with transplant regulatory requirements and reporting agencies.
  • Knowledge of UNOS processes, transplant protocols, and quality improvement methodologies.

Compensation:

Salary Range: $39.48 - 65.14 per hour 

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
