Location: San Antonio, TX and surrounding areas Job Type: Full-time

 

Job Summary

The Nephrology Nurse Practitioner (NP) provides advanced, patient-centered care to individuals with acute and chronic kidney disease across inpatient, outpatient clinic, and dialysis settings. The NP collaborates with nephrologists and interdisciplinary teams to optimize outcomes, manage complex renal conditions, and promote kidney health.

 

Key Responsibilities

Clinical Care

 

  • Perform comprehensive assessments, including history, physical exams, and risk evaluation for kidney disease.
  • Diagnose and manage acute and chronic renal conditions, including but not limited to:
    • Chronic kidney disease (CKD)
    • End-stage renal disease (ESRD)
    • Acute kidney injury (AKI)
    • Glomerulonephritis and nephrotic syndrome
    • Electrolyte and acid-base disorders
  • Order, interpret, and analyze diagnostic studies (labs, imaging, biopsies).
  • Develop and implement individualized treatment plans.
  • Initiate and manage medications (e.g., antihypertensives, diuretics, immunosuppressants, SGLT2 inhibitors, etc.).

Dialysis Management

 

  • Manage patients receiving hemodialysis and peritoneal dialysis.
  • Monitor dialysis adequacy, tolerance, and complications.
  • Collaborate with dialysis teams to adjust treatment prescriptions.
  • Address access-related issues (AV stula, grafts, catheters) in coordination with nephrology and vascular surgery teams.

Inpatient & Outpatient Rounding

  • Round on inpatient census in hospitals and rehab/SNF facilities for nephrology consults and follow-up.
  • Provide consultations for renal-related conditions to hospital and rehab teams.
  • Manage transitions of care, including discharge planning and communication with outpatient providers.
  • Conduct outpatient clinic follow-ups and chronic disease management visits. Patient Education & Counseling
  • Educate patients and families on:
    • Disease progression and prevention
    • Medication adherence
    • Diet and fluid management
    • Dialysis options and transplant considerations
  • Promote lifestyle modications to slow CKD progression and improve overall health. Care Coordination
  • Collaborate with nephrologists, primary care providers, hospitalists, dietitians, social workers, and dialysis staff.
  • Facilitate referrals (transplant, vascular surgery, specialty care).
  • Ensure continuity of care across inpatient, outpatient, and dialysis settings. Quality & Compliance
  • Participate in quality improvement initiatives, including Monthly Continuous Quality Initiative (CQI) meetings with group providers.
  • Ensure adherence to regulatory standards and practice protocols.
  • Maintain documentation in accordance with clinical, legal, and billing standards.

 

Qualications

  • Current Texas Nurse Practitioner license (or eligibility to obtain).
  • Board certification as an NP or DNP (ANCC or AANP).
  • DEA license and prescriptive authority in Texas (or eligibility).
  • Minimum 1–2 years of clinical experience preferred but not required; dialysis, nephrology, or internal medicine experience strongly preferred.

 

Preferred Experience

  • 1–3 years nephrology or related experience (ICU, dialysis, internal medicine).
  • Familiarity with ESRD quality measures and CMS regulations.
  • Experience with EMR systems.

Skills

  • Strong clinical judgment and advanced diagnostic interpretation.
  • Effective communication with patients, families, and interdisciplinary teams.
  • Interdisciplinary collaboration across hospital, clinic, and dialysis settings.
  • Strong organizational skills and attention to detail.

 

Work Environment

  • Mixed practice across hospital, outpatient clinic, rehab/SNF, and dialysis unit settings with frequent travel between locations in the San Antonio area.

 

Schedule and Coverage

  • Full-time position.
  • Travel between multiple dialysis centers and practice locations in San Antonio and surrounding areas.
  • Rotating weekend call schedule with supervising physician assistance.
  • Rotational holiday coverage with supervising physician assistance.
  • Participation in monthly CQI meetings with group providers.
