Ritter Center is currently recruiting for a Program Manager for our Case Management Department. This full-time, exempt position reports to the Chief of Staff based in San Rafael, CA; the role operates out of our main Campus.
The Case Management Program Manager is an integral role in Ritter Center’s dynamic multidisciplinary team of medical, behavioral health and case management professionals who provide integrated care to our target population. This position reports directly to the Chief of Staff and supervises a team of 7 employees comprised of 3 Case Managers and 3 Patient Navigators and 1 Admin Coordinator. The Program Manager also provides a limited amount of direct client services while managing the work of the team. The services are: Representative Payee, Patient Navigation, Rapid Rehousing funds, Housing First (HBCM), and Custody to Community. This position also serves as the Agency Lead for the Homeless Management Information System (HMIS) to ensure timely and accurate data entry across all agency HMIS users. This role leads efforts to triage all drop-in case management and referral services and oversees the coordination of overall client flow.
Essential Job Functions and Responsibilities:
- Recruit, hire, train and supervise a team of Case Managers and Patient Navigators providing trauma-informed care across a comprehensive and diverse range of programs to ensure effective services and outcomes.
- Conduct regular team meetings and individual supervision sessions.
- Oversee the day-to-day operations, service quality, and client flow of Case Management programs while ensuring compliance with all legal, regulatory, contractual, and funding requirements.
- Meet regularly with Chief of Staff and attend staff meetings and training as scheduled.
- Function as a part of a multidisciplinary team to support the engagement and services for chronically homeless individuals and families with severe and persistent mental illness, physical disabilities, and/or substance use.
- Collaborate with Accounting to verify compliance and accurate tracking of Housing First rental subsidy payments, Rapid Rehousing payments, Rental Assistance payments, Representative Payee accounts management and Social Security reporting.
- Attend and represent Ritter Center at all required County level case conferencing, advocacy, and policy meetings as well as community events and media appearances when requested.
- Oversee accurate record-keeping, documentation, reporting, and retention of client and program records within HMIS and other organizational systems.
- Partner with the Clinical staff to ensure integration of clinical services.
- Supervise and provide strategic direction for case management staff, including leading meetings, performance evaluations, and professional coaching.
- Ensure effective onboarding, performance management, and policy adherence of staff supervised.
- Maintain high staff morale to enable a positive and inclusive culture.
- Work collaboratively to enhance service integration within the organization and non-profit community partners throughout Marin County.
- Serve as a member of the leadership team. Contribute to the development of and adhere to Ritter Center’s goals, objectives, policies and procedures.
- Support Quality Improvement (QI) activities; collect, analyze and report data regarding client services according to all case management contracts as well as organizational-wide QI measures.
- Collaborate closely with Marin County Continuum of Care (CoC) partner Community Based Organizations (CBO), and other Social Service agencies to ensure appropriate support and referrals are being made by the team.
- Oversee resolution of complex service challenges and guide staff in developing effective approaches and solutions.
Minimum Qualifications
- Per our Federally Qualified Healthcare Center (FQHC) requirements employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being vaccinated against COVID-19 (unless applying for an exemption) per the August 5, 2020 State Public Order for Healthcare Facilities, providers and employees.
- Bachelor's degree in Psychology, Social Work, Counseling, Criminal Justice, or another behavioral or social science field.
- Equivalent experience (2+ years) in benefits administration, Representative Payee services, case management, or a closely related field. Additional experience may be substituted for a degree.
- Experience with social services in Marin County is highly preferred.
- Prior management experience in a social services setting, including employee supervision, implementation of new service models, program management, and ensuring compliance with contractual and regulatory requirements.
- Familiarity with budgeting and accounting practices is highly preferred.
- Knowledge of public assistance and social service programs, public housing systems, HUD regulations (including 24 CFR Parts 576 and 578), Housing First principles, housing-based case management practices, and Fair Housing requirements.
- Understanding of Social Security regulations, policies, and procedures, including 42 U.S.C. §§ 1007-1008 and 20 CFR Part 1, Subpart F.
- Working knowledge of public benefits programs, including Medi-Cal and CalFresh, and experience supporting individuals with disabilities.
- Knowledge of health care systems, social determinants of health, and housing and supportive service programs.
- Effective verbal and written communication skills, including the ability to communicate clearly, de-escalate individuals in crisis, and interpret social service programs and resources for applicants and participants.
- Experience working with individuals experiencing homelessness, trauma, disabilities, behavioral health conditions, and other complex needs in highly vulnerable circumstances.
- Proficiency in documentation, case note management, database systems, and Microsoft Office Suite.
- Strong organizational skills, attention to detail, and ability to manage multiple priorities.
- Experience using Motivational Interviewing and other engagement strategies with homeless and/or dually diagnosed populations.
- Ability to work effectively with individuals from diverse cultural, racial, educational, and socioeconomic backgrounds.
- Bilingual English/Spanish preferred but not required.
- Must possess a valid California driver's license with a satisfactory driving record.
