Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Primary Care Physician for Element Care PACE in Lynn, Massachusetts.
POSITION OVERVIEW
- Provide comprehensive primary care for medically complex older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).
- Manage acute illnesses, chronic diseases, preventive care, medication management, and annual wellness evaluations.
- Conduct visits in the PACE center, participant homes, skilled nursing facilities, and through telehealth when appropriate.
- Serve as the physician leader within an interdisciplinary care team alongside Nurse Practitioners, nurses, therapists, social workers, pharmacists, and care coordinators.
- Complete hospital follow-up visits and coordinate care transitions to reduce avoidable admissions.
- Support participant-centered goals that allow seniors to remain safely living within the community.

- Competitive salary from $200,000-$294,000 based on experience.

Requirements
MINIMUM REQUIREMENTS
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Eligible for unrestricted Massachusetts medical license.
- Active DEA registration or ability to obtain.
- Experience or strong interest in geriatrics, primary care, or complex chronic disease management.
- Excellent communication and interdisciplinary collaboration skills.
- Commitment to patient-centered, community-based senior care.
