About the Role
The Acute Care Transition Coordinator (ACTC) is responsible for the coordination of bed management in the Acute Mental Health Inpatient Service (AIS). The position will coordinate bed flow to and from the AIS ensuring maximum efficiency around bed availability to support the organisation to achieve KPIs and government targets including NEAT.

Classifications for this position Registered Psychiatric Nurse Grade 5 (NP51).

Your Contribution

• Degree or postgraduate diploma in psychiatric/mental health nursing

• Completed or working towards a higher degree in nursing or relevant area

• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) Division 1

• Demonstrated ability to provide support, mentoring, training and education to students, graduates and early career nurses

• Minimum 5 years post qualification experience within mental health services.

About Mental Health
St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner-city areas of Yarra and Boroondara. St Vincent’s Mental Health practices with a recovery orientated approach. This position, through day-to-day care work and other activities, will actively contribute to the development and implementation of this model.
