Your Opportunity 

Northern Sydney Homelessness Services provides case management and advocacy support to people experiencing homelessness or at risk of homelessness.

As a Case Manager, you will provide person-centred support to help individuals explore housing options, address mental health and alcohol and other drug (AOD) concerns, and build the connections needed to achieve long-term independence and stability.

Your key responsibilities will be to:

• Respond to referrals and complete comprehensive assessments to determine eligibility and support needs.
• Develop, implement and regularly review individualised case plans in collaboration with clients and key stakeholders.
• Support clients across a range of areas including housing, health, education, employment, life skills, family relationships and community participation.
• Provide specialist advice and maintain strong relationships with local service providers and community networks.
• Build effective partnerships with government agencies, community housing providers, allied health professionals and other stakeholders.
• Contribute to program reporting, data collection, outcomes measurement and continuous improvement activities.

Requirements for Success

• A relevant Degree or Diploma qualification with at least 1–2 years' experience
• Experience working with people experiencing homelessness and related challenges such as poverty, trauma, mental health concerns, domestic and family violence, or alcohol and other drug issues.
• Demonstrated ability to manage a complex caseload and deliver specialised or therapeutic interventions.
• Strong administrative, written and verbal communication skills.
• An understanding of recovery-oriented practice gained through lived experience and/or professional development.
• Ability to build positive stakeholder relationships and work collaboratively across services.
• Current Driver's Licence.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
